Benefit of upfront combination lipid-lowering therapy in statin-naïve acute coronary syndrome patients with intermediate admission LDL-C and Lp(a)-driven predicted monotherapy failure

The 2025 ESC/EAS Focused Update on the Management of Dyslipidaemias recommends upfront combination therapy (high-intensity statin + ezetimibe) during index hospitalization for acute coronary syndrome (ACS) in lipid-lowering therapy (LLT)-naïve patients unlikely to achieve LDL-C goals with monotherapy. Elevated li...

Association of apolipoprotein B and lipoprotein(a) with severe atherosclerotic cardiovascular disease in patients with acute coronary syndrome: the modifying effect of prior lipid-lowering therapy.

The interplay between apolipoprotein B (apoB) and Lipoprotein(a) [Lp(a)] in determining atherosclerotic cardiovascular disease (ASCVD) severity remains complex, particularly under the influence of lipid-lowering therapy (LLT). To evaluate the association of apoB and Lp(a) with severe ASCVD in patients hosp...

Upfront combination lipid-lowering therapy in statin-naive acute coronary syndrome patients with high admission LDL-C: real-world support for the 2025 ESC dyslipidaemias guidelines

The findings strongly support the 2025 ESC/EAS Guidelines update: in patients with LDL-PMF, adding ezetimibe to high-intensity statins at discharge significantly increases the likelihood of achieving the <55 mg/dL target compared to monotherapy.
