HIV-1 integrase inhibitor resistance in viraemic-treated individuals and baseline capsid diversity relevant to future lenacapavir therapy

These findings demonstrate DTG resistance in both adult and paediatric participants with virological failure with G118R emerging as a prominent resistance pathway and support continued surveillance of integrase and capsid resistance as long-acting antiretroviral strategies expand.

Fixation of E340D abolished the surrogate defence provided by sotrovimab, eliminating neutralizing activity and exerting a positive epistatic effect on emerging mutations, which conferred the highest infectivity across all cell lines, with the most significant effect in low-TMPRSS2 cells.
