Abstract

Introduction: Sclerosing mucoepidermoid carcinoma with eosinophilia of the thyroid is a rare malignant neoplasm, typically associated with chronic lymphocytic thyroiditis. It usually presents as a painless, slowly enlarging unilateral thyroid nodule, frequently mimicking a benign lesion. Case report: A 77-year-old woman presented a large fast-growing thyroid mass. Fine-needle aspirates rendered a diagnosis of atypia of undetermined significance. Histopathology revealed proliferation of solid and pseudoangiomatous nests of cells showing predominant squamous differentiation, with focal mucinous cells, in a sclerotic and eosinophil-rich stroma. Areas of high-grade morphology were observed. The tumor was diffusely positive for AE1/AE3, CK5/6, p63, and p40, and focally positive for CK7, CEA, and TTF1. Ki-67 was heterogeneous. The final diagnosis of sclerosing mucoepidermoid carcinoma with eosinophilia with high-grade transformation was established. Next generation sequencing panel showed high levels of NTRK1 expression and copy number alterations on VHL gene locus of chromosome 3, with no other mutations or fusions known to occur in thyroid cancer. The patient had a local recurrence of the high-grade component, followed by lung and skull metastasis, and died from the disease. Conclusion: The current case is remarkable due to the high-grade transformation and aggressive clinical course. The features discussed herein intend to help the further characterization of sclerosing mucoepidermoid carcinoma with eosinophilia of the thyroid, especially its molecular background.

High-grade Pulmonary Mucoepidermoid Carcinoma in a Child: A Rare Case Report

Mucoepidermoid Carcinoma (MEC) of the lung is a rare salivary gland-type tumour, accounting for <1% of primary pulmonary neoplasms, with female predominance in some series and a wide age range. This case reports a four-year-old girl with a four-month history of cough, haemoptysis, and breathlessness. Imaging revealed a...

Breast Adenomyoepithelioma Mimicking Fibroadenoma on Cytology: A Diagnostic Pitfall

Background: Adenomyoepithelioma (AME) of the breast is a rare biphasic neoplasm composed of epithelial and myoepithelial cells. Its diverse morphology frequently leads to diagnostic challenges, particularly on cytology where it may mimic benign fibroepithelial lesions. Case Presentation: We report a case of breast aden...

Sclerosing Polycystic Adenoma Masquerading as Malignancy on Fine Needle Aspiration Cytology.

Sclerosing polycystic adenoma (SPA) is a rare benign salivary gland neoplasm composed of a lobular proliferation of ducts lined by cuboidal, columnar, apocrine, and foamy vacuolated cells, and acinar structures having large cells with intracytoplasmic bright eosinophilic granules in a collagenous stroma with lymphoid a...

Pilomatrix-Like High-Grade Endometrial Carcinoma (PiMHEC) With Secondary Vaginal Involvement: A Rare Variant of CTNNB1-Mutated Endometrial Carcinoma Mimicking Mesonephric-Like Adenocarcinoma

Pilomatrix-like high-grade endometrial carcinoma (PiMHEC) is a recently recognized, clinically aggressive variant of endometrial carcinoma that is frequently misdiagnosed due to its morphological and immunophenotypic overlap with other high-grade endometrial malignancies. We report a 52-year-old woman who presented wit...

Nasal polyp with bizarre stromal cells mimicking malignancy: a diagnostic pitfall

Bizarre stromal cells in sinonasal polyps represent a rare reactive phenomenon that may closely mimic malignancy and lead to diagnostic pitfalls. We report the case of an 18-year-old male presenting with progressive unilateral nasal obstruction and a history of prior endoscopic sinus surgeries. Imaging revealed a large...
