Abstract
Special article for a monograph on Clinical Decision Support Systems (CDSS). Clinical decision support systems are moving from calculators and alerts to machine-learning and large-language-model assistants. NeonatAIlogy™ is the discipline of bringing such artificial intelligence safely into newborn care, education, research and governance. This article sets out its scope, where CDSS genuinely help in the NICU, why staged validation is non-negotiable (a seven-rung validation ladder), how innovators should sequence intellectual-property protection, regulation, publication and presentation, and a balance sheet of utility, limitation and remarks. It closes with ten pearls for neonatal innovators and a summary of how the chapter serves learners, NICU teams, innovators, families and policy-makers. Includes three original diagrams, four tables and 31 verified references (checked against PubMed and Zenodo records on 27 Sep 2026). AI-use declaration: drafting was supported by Claude (Anthropic) and literature synthesis by Consensus AI; the author reviewed all content and is responsible for it. References 19 and 31 are the author's own, not externally validated resources. Not a substitute for expert clinical judgement, legal or regulatory advice.
The results are packaged in the Greenfield Startup Model (GSM), which explains the priority of startups to release the product as quickly as possible, and the need to shorten time-to-market, by speeding up the development through low-precision engineering activities.

The results indicate that software developers are a slightly happy population, but the need for limiting the unhappiness of developers remains, and 219 factors representing causes of unhappiness while developing software are identified.
