In this single case, LCT with a de‐epithelialized free gingival graft was associated with stable soft tissue coverage and favorable esthetic and patient‐reported outcomes over 24 months; further studies are required to determine reproducibility and predictability.

Abstract

Peri‐implant soft tissue dehiscence is an esthetic complication for which coronally advanced flap with connective tissue grafting remains a predictable treatment, whereas evidence for laterally closed tunneling (LCT) around implants is limited. This single‐patient case report describes a 4 × 3 mm labial peri‐implant soft tissue dehiscence at an immediately placed implant replacing the maxillary left central incisor. LCT combined with a de‐epithelialized free gingival graft and limited implantoplasty was performed. At 24 months, complete coverage was maintained; buccal soft tissue thickness increased from 1.0 to 2.7 mm, keratinized tissue width from 1 to 3 mm, Pink Esthetic Score from 6 to 12, Mucosal Scarring Index from 3 to 0, and patient satisfaction from 4 to 10. Standardized periapical radiographs showed stable interproximal crestal bone levels without evidence of progressive bone loss. In this single case, LCT with a de‐epithelialized free gingival graft was associated with stable soft tissue coverage and favorable esthetic and patient‐reported outcomes over 24 months; further studies are required to determine reproducibility and predictability.

Posterior Maxillary Implant Rehabilitation Using Staged Ridge Preservation and Soft‐Tissue Augmentation: A 2‐Year Case Report

Background Rehabilitation of the posterior maxilla is challenging due to sinus pneumatization, limited vertical bone height, and thin peri‐implant soft tissues. Minimally invasive, biologically guided approaches aim to reduce morbidity while ensuring predictable outcomes. This report presents a staged protocol integrat...

Soft Tissue Management in Implant Rehabilitation After Vascularized Free Bone Flap Jaw Reconstruction: A Narrative Review

Jaw reconstruction with a vascularized free bone flap has significantly improved outcomes for patients requiring maxillofacial rehabilitation by restoring oral function and esthetics and improving psychological well-being. However, the long-term success of dental implants in reconstructed jaws depends on the health o...

Subperiosteal circumferential connective tissue graft for vertical implant soft tissue augmentation.

BACKGROUND Vertical implant soft tissue augmentation is indicated to optimize esthetic outcomes in cases of peri-implant soft tissue deficiency or dehiscence, particularly when the peri-implant mucosa is exposed during a full smile. This manuscript describes the use of a subperiosteal circumferential connective tissue...

Dimensional Changes of the Peri-Implant Mucosa Applying a Suture-Free Wound Closure Using a Healing Abutment Attachment: A Randomised Controlled Clinical Trial.

AIM To compare peri-implant soft-tissue dimensional changes following wound closure using a suture-free healing abutment attachment (HAA) compared to conventional suturing during implant surgery. MATERIALS AND METHODS Thirty patients who were randomly allocated to the HAA group (15 patients, 20 implants) or the sutur...
