Abstract

Gastric involvement with Histoplasma capsulatum infection is rare but can mimic peptic ulcers or cancer, delaying diagnosis. A 71-year-old man presented with oral ulcer, abdominal pain, and weight loss. The patient had history of tonsillar histoplasmosis treated 8 years before the current presentation with confirmed resolution. The patient had no history of malignancy, autoimmune diseases, or recent travel. Initial laboratory evaluation revealed normocytic anemia. Imaging did not show acute abnormalities that would account for his symptoms. Esophagogastroduodenoscopy revealed multiple gastric ulcers. Biopsies showed chronic gastritis with ulceration and fungal yeasts consistent with H. capsulatum. Similarly, the oral ulcer showed non-necrotizing granulomas with fungal forms. Itraconazole was initiated and follow-up esophagogastroduodenoscopy showed healing ulcers.

Disseminated histoplasmosis presenting with polymorphic mucocutaneous lesions in a renal transplant recipient

Disseminated histoplasmosis is an opportunistic fungal infection caused by Histoplasma capsulatum that commonly affects immunocompromised individuals. Cutaneous manifestations are polymorphic and may mimic several dermatological disorders, leading to diagnostic difficulty. We report a 33-year-old renal transplant r...

Isolated gastric Langerhans cell histiocytosis: a case report and literature review

Langerhans cell histiocytosis (LCH) is a rare histiocytic disease that predominantly affects young children, with adult cases being exceedingly uncommon. The most frequently affected sites in LCH are bones, lungs, and the pituitary gland, in whom gastrointestinal involvement is scarce. Primary isolated gastric LCH is r...

Disseminated Nocardia pseudobrasiliensis infection in a case of juvenile systemic lupus erythematosus on immunosuppressive therapy

Nocardiosis is an uncommon infection that presents as a chronic, debilitating illness with radiographic manifestations simulating lung cancer or tuberculosis. Immunocompromised hosts may develop a fulminant disease resembling acute bacterial pneumonia or disseminated disease. We report a case of a juvenile lupus who de...

Idiopathic esophageal ulcer associated with HIV: a forgotten entity in patients with acquired immunodeficiency syndrome

Esophageal ulceration in patients with stage 3 HIV infection may have infectious, inflammatory, neoplastic, chemical, or irritative etiologies, and their behavior has  changed with the advent of highly active antiretroviral therapy. We report the case of a 33-year-old patient with a recent diagnosis of stage 3 HIV who...

Pseudotumoral donovanosis mimicking squamous cell carcinoma in a patient with HIV infection

Donovanosis is a chronic granulomatous sexually transmitted infection that may clinically mimic malignancy. We report a 64-year-old man with well-controlled HIV infection presenting with a chronic infiltrated vegetative scrotal ulcer showing progressive enlargement, exophytic growth, and hemorrhagic ulceration. Despite...

Gastrointestinal Histoplasmosis Mimicking Intestinal Tuberculosis and Crohn's Disease: Ileocecal Involvement in an Immunocompetent Host

Gastrointestinal histoplasmosis is an under‐recognized manifestation of disseminated Histoplasma capsulatum infection. Its ileocecal predilection closely mimics intestinal tuberculosis (TB) and Crohn's disease, posing serious diagnostic challenges, particularly in TB‐endemic regions. A 26‐year‐old immunocompetent male...
