Abstract
A 19-year-old male presented with secondary hemophagocytic lymphohistiocytosis, persistent fever, abdominal pain and melaena. His clinical course was complicated by gram-negative sepsis (Klebsiella NDM-positive and E coli), Clostridioides difficile infection, cytomegalovirus (CMV) pneumonia and Elizabethkingia anophelis infection, leading to multiorgan dysfunction syndrome and massive gastrointestinal bleeding. Diagnosis was confirmed via bone marrow biopsy and relevant imaging. Management included HLH-2004 immunochemotherapy, broad-spectrum antimicrobials, ICU support and blood transfusions. This report underscores the diagnostic difficulty, rapid deterioration and high mortality risk when HLH coexists with opportunistic infections.
Gastrointestinal mucormycosis is a rare but life-threatening invasive fungal infection affecting immunocompromised individuals. Emphysematous gastritis is a rare, fulminant manifestation with high mortality. We report a case of a woman in her 80s with multiple comorbidities, including diabetes mellitus and necrotizin...

Hyperleukocytosis (> 100 × 109/L) is typically associated with hematologic malignancies and is rarely reported in infection. A previously healthy 27‐year‐old man presented with acute gastrointestinal symptoms and was found to have meningococcal bacteremia complicated by thrombocytopenia, coagulopathy, and acute kidney...

Hemophagocytic lymphohistiocytosis (HLH) is a severe inflammatory response syndrome. Common infectious triggers include Epstein Barr virus, cytomegalovirus, bacteria, and other pathogens. We report a case of HLH caused by human herpesvirus-6 (HHV-6) in a pediatric patient. Based on the HLH-2004 diagnostic criteria, the...

X-linked agammaglobulinemia (XLA) is a rare immunodeficiency caused by a mutation in Bruton's tyrosine kinase, resulting in profoundly low immunoglobulin levels and susceptibility to recurrent infections. We report a 14-year-old patient with XLA who presented with persistent fever, bloody diarrhea, and headache suggest...

Background and Clinical Significance: Secondary hemophagocytic lymphohistiocytosis (HLH) is a life-threatening hyperinflammatory syndrome that can closely resemble sepsis in adults, potentially delaying recognition and treatment. Because infection may trigger or coexist with HLH, persistent or disproportionate inflamma...
