TL;DR

The Nuts Holder is a safe and feasible device that standardizes tissue fixation, prevents assistant-related instability, and successfully facilitates solo surgery in urology.

Abstract

NeedAdequate stabilization in scrotal and penile surgeries is essential but traditionally relies on manual fixation, causing assistant fatigue and operative instability.Technical SolutionWe developed the Nuts Holder, an adjustable, reusable stainless steel support device featuring a screw-and-slide mechanism designed to gently clamp and stabilize the target organ.Proof of ConceptIn 16 consecutive urological cases, including microTESE, the device was successfully applied without any complications. A surgeon survey demonstrated 100% agreement on improved operative stability, reduced assistant dependence, and enhanced maintenance of microscopic focus.Next StepsTo address an initial learning curve regarding setup ease, a second-generation model featuring a quick-release mechanism and reduced weight is currently under development.ConclusionThe Nuts Holder is a safe and feasible device that standardizes tissue fixation, prevents assistant-related instability, and successfully facilitates solo surgery in urology.

Zero-touch technique: a new paradigm in penile prosthesis surgery enabled by a sterile genital barrier system.

Penile prosthesis (PP) implantation remains the most definitive treatment for medication-refractory erectile dysfunction (ED). Despite reductions in infection rates achieved through improved PP coatings, antimicrobial protocols, and refined surgical techniques, PP infection continues to impose clinical and economic bur...

The hidden costs of technical advancements: software and equipment troubleshooting in robotic inguinal hernia repair

Abstract While robotic transabdominal preperitoneal (R-TAPP) hernia repairs offer excellent visibility, literature overlooks time lost to technical troubleshooting. This technical note evaluates a 30-minute pre-incision delay during a routine R-TAPP procedure, precipitated by a software synchronization error and system...

A low-cost indigenous self-retaining perineal retractor: Design and early clinical evaluation.

Commercial self-retaining perineal retractors provide excellent exposure during anorectal and vaginal surgery but remain prohibitively expensive for many resource-limited hospitals. We designed an indigenous self-retaining perineal retractor using readily available materials, incorporating a re-usable stainless-steel r...

Bimanual instrument suturing technique for periodontal and implant microsurgery: A technical note.

BACKGROUND In the traditional suturing technique, the dominant hand (DH) holds a needle holder, and the non-dominant hand (NH) performs manual ties without the aid of an instrument. In periodontal and implant microsurgery, finer sutures and needles, e.g., 7-0 in International Organization for Standardization units, are...

Technical Modifications to Improve Rubber Dam Isolation: Clinical Tips

Rubber dam isolation is fundamental to achieving an aseptic operative field, protecting the patient’s airway, and facilitating restorative and endodontic procedures. However, clinicians may encounter practical difficulties related to hole positioning in heavy-gauge and opaque rubber dam sheets, tearing of the dam, inad...
