Abstract

The transition to digital health information systems is crucial for ensuring the quality of clinical services at primary healthcare facilities (Nurcahyanti, 2025). This study aimed to analyze the implementation of Electronic Medical Records (EMR) through the Puskesmas Management Information System (SIMPUS) at Puskesmas Umbulharjo I Yogyakarta using the HOT-Fit framework. A descriptive qualitative method with a cross-sectional design. Data were gathered through in-depth interviews with seven user informants and one triangulation informant, supported by direct observation and document reviews. The human component revealed high user satisfaction and full system utilization, although legally required clinical features remain unavailable. Evaluation of the organizational dimension demonstrated clear decree and SOP structures, yet routine training is lacking and hardware specifications remain suboptimal. Technically, the core system quality is stable, with operational disruptions predominantly triggered by internet bandwidth constraints and BPJS P-Care bridging errors. Nevertheless, the net benefits show highly positive outcomes, significantly accelerating patient services, eradicating missing physical charts, and eliminating double-entry tasks. In conclusion, EMR implementation performs optimally regarding system utilization and organizational structures, but critically requires infrastructure reinforcement, continuous training, and expanded clinical features.
