This review will analyze the current evidence supporting established predictive biomarkers, including PD-L1 expression, while highlighting their major limitations, and discuss and summarize emerging clinical and molecular biomarkers and integrative predictive approaches aimed at improving the stratification of patients with NSCLC.
Abstract
Immune checkpoint inhibitors (ICIs) have radically changed the treatment landscape of non-small cell lung cancer (NSCLC) and significantly improved patients' prognosis in many settings of the disease. Nevertheless, clinical outcomes remain highly heterogeneous, and a substantial proportion of patients do not achieve meaningful and/or durable benefit from immunotherapy. This variability in treatment response has highlighted the need to identify more robust predictive biomarkers that integrate, beyond established biomarkers such as programmed cell death ligand 1 (PD-L1) expression, additional clinical, genomic, immunologic, and microenvironmental features to better identify the biological determinants underlying sensitivity and resistance to ICIs. In this review, we will analyze the current evidence supporting established predictive biomarkers, including PD-L1 expression, while highlighting their major limitations. We will also discuss and summarize emerging clinical and molecular biomarkers and integrative predictive approaches aimed at improving the stratification of patients with NSCLC, to identify those most likely to benefit from ICI therapy and ultimately refine personalized treatment strategies.
The introduction of immune checkpoint inhibitors has transformed therapeutic strategies for non-small cell lung cancer; however, the generalisation of data regarding their molecular mechanisms of action and predictors of therapeutic efficacy remains an important challenge. The aim of this study was to conduct a compreh...

Immune checkpoint inhibitors (ICI) have revolutionized the treatment of TNBC, leading to durable responses and improved survival. With the adoption of immunotherapy into high-risk early-stage and selected metastatic triple-negative breast cancer (TNBC), clinicians’ ability to recognize and treat immune-related adverse...

Although immunotherapy has completely transformed the treatment landscape of nonsmall cell lung cancer (NSCLC), its wide application is still limited by the heterogeneity of patient responses, primary and acquired resistance, as well as the management challenges of immune‐related adverse events (irAEs). To achieve prec...

Characteristics of the tumor immune microenvironment and PET imaging of PD-1/PD-L1 have potential as biomarkers for immunotherapy in NSCLC are demonstrated and larger studies are required to validate the predictive value of these approaches.
The need for comprehensive molecular characterization and integrated biomarker-guided treatment strategies to improve therapeutic decision-making and clinical outcomes in lung adenocarcinoma is highlighted.
