Comparative analysis of the incidence and disability of high blood pressure diseases and the main associated diseases in the administrative district of the megapolis
Abstract
Introduction. Diseases associated with high blood pressure, like circulatory system diseases in general, is the leading cause of morbidity and disability in both Russia and St. Petersburg. While there are no studies on the dynamics of circulatory system diseases morbidity and disability at the administrative district level, they are necessary for the development of an effective regional model for providing medical care to individuals with circulatory system diseases. Aim. To study the territorial and administrative features of morbidity and disability indicators when organizing a territorial model for providing medical care to people with circulatory system diseases. Materials and methods. Official statistics were analyzed. Extensive and intensive indicators for 2019–2024 were calculated and analyzed, and a linear regression analysis was conducted. Results. Сirculatory system diseases remain the leading causes of overall and primary morbidity and disability in all analyzed areas. A statistically significant increase in the primary incidence of diseases associated with high blood pressure in the Kalininsky District of St. Petersburg, consistent with the Russia, was observed for both working-age and older individuals, which may indicate, among other things, an improvement in the detection and screening system. An analysis of the statistically significant excess of primary incidence of acute cerebrovascular accident in the Kalininsky district compared to St. Petersburg showed statistically significant differences between medical organizations in the Kalininsky district: against the background of the same gender and age composition, housing characteristics, this may be due specifically to the organization of medical care for patients with acute cerebrovascular accident. Conclusion. The overall and primary incidence and disability rates due to circulatory system diseases are influenced by the coverage and comprehensiveness of medical screening, as well as the implementation of organizational and management decisions when working with patients with modifiable risk factors for the development and progression of cardiovascular diseases, and the effectiveness of outpatient medical rehabilitation.
