TL;DR

The results show that daraxonrasib prolonged survival, slowed disease progression and led to tumour shrinkage more frequently than chemotherapy, whilst causing mainly mild, manageable side effects.

Abstract

Simple Summary Pancreatic cancer is one of the most aggressive forms of cancer, and in the majority of cases involves a mutation in the RAS gene, a protein which, when activated, stimulates tumour growth. Inhibiting this protein has long been a challenge, and previous drugs were effective against only one rare type of mutation. In August 2026, a new oral drug called daraxonrasib was approved, as it is capable of blocking the active form of RAS across multiple mutation types, offering hope to a greater number of patients. This review brings together the available data from clinical trials on the efficacy and safety of daraxonrasib in patients whose pancreatic cancer has progressed following prior treatment. The results show that daraxonrasib prolonged survival, slowed disease progression and led to tumour shrinkage more frequently than chemotherapy, whilst causing mainly mild, manageable side effects. These results confirm that daraxonrasib is a promising new drug and also highlight areas where further research is still needed, particularly with regard to rarer types of mutations.

Daraxonrasib: The first-in-class RAS(ON) multi-selective inhibitor approved for treatment of metastatic pancreatic adenocarcinoma.

Advanced pancreatic ductal adenocarcinoma (PDAC) has a poor prognosis, with limited benefits from conventional chemotherapy. Daraxonrasib is an oral, noncovalent RAS(ON) multi-selective inhibitor that forms a ternary complex with cyclophilin A and GTP-bound RAS, blocking downstream signaling across multiple mutant and...

Abstract IA012: A New Perspective: Clinical Trials in the Era of RAS Inhibitors

The demonstration of an overall survival benefit for RAS(ON) multi-selective inhibition in previously treated metastatic pancreatic ductal adenocarcinoma (RASolute 302) establishes the first randomised evidence for RAS-directed therapy in this disease. Previously, biomarker-directed treatment in PDAC reached a minori...

RAS-targeted therapies for pancreatic cancer

Pancreatic adenocarcinoma (PDAC) is a RAS-driven cancer with poor prognosis. Although tremendous progress has been made in other cancers with the development of targeted therapies and immunotherapy, cytotoxic chemotherapy remains the mainstay of treatment for PDAC. For decades, the therapeutic development of RAS-target...

Abstract B074: Comparative analysis of next-generation KRAS inhibitors in patient derived pancreatic cancer organoids

Pancreatic ductal adenocarcinoma (PDAC) is one of the deadliest cancers with a five-year survival rate below 10%. Due to aggressiveness and lack of early detection methods, patients are often diagnosed at an advanced stage, leaving few therapeutic options. This emphasizes the need for a deeper understanding of the mo...
