TL;DR

The findings from this study demonstrate the feasibility of individual, parcel-guided rTMS and suggest response/remission rates similar to those observed using individualized fMRI localization of the peak anti-correlated region.

Abstract

Efficacy of Transcranial Magnetic Stimulation (TMS) for depression has been shown to correlate with the proximity of stimulation to the region of LDLPFC most anti-correlated with subgenual anterior cingulate cortex (sgACC). Using the Human Connectome Project (HCP) surface-based reconstruction from a structural MRI, we observed that the region of DLPFC most anti-correlated with sgACC falls in anterior parcel 46 and encompasses the junction between adjacent parcels. Individual parcel mapped TMS directed at this target showed high efficacy for treatment resistant depression in a group of 15 patients. Pre-treatment anti-correlation between DLPFC (parcel 46) and sgACC correlated significantly with treatment response. These findings, along with our prior study, demonstrate the feasibility of individual, parcel-guided rTMS and suggest response/remission rates similar to those observed using individualized fMRI localization of the peak anti-correlated region.

Excitation-Inhibition Balance and Fronto-Limbic Connectivity Distinguish TMS Treatment Outcomes in Refractory Depression.

BACKGROUND Intermittent theta burst stimulation (iTBS) targeting the left dorsolateral prefrontal cortex (DLPFC) is effective for treatment-resistant depression, but outcomes remain variable. The neurophysiological mechanisms underlying response remain poorly understood, limiting precision neuromodulation. METHODS We...

Treatment response prediction in major neurocognitive disorder with depression using the scalp-to-cortex distance of TMS targets: implications for personalized dose estimation

Predictive models integrating with MRI-derived SCD and SCD-MOOD showed strong specificity for active rTMS compared to sham stimulation, and may enable the development of effective, personalized protocols to improve region-specific TMS treatment outcomes in clinical practice.

Toward an Iterative, Testable Transcranial Magnetic Stimulation Framework for Posttraumatic Stress Disorder

Transcranial magnetic stimulation (TMS) has shown promise as a treatment for posttraumatic stress disorder (PTSD); however, the field continues to face a deceptively simple question: where should stimulation be delivered? Existing studies have targeted the left dorsolateral prefrontal cortex (DLPFC), right DLPFC, media...

Accelerated TMS is associated with changes of subgenual cingulate-default mode network coupling in depression: an observational 7 Tesla fMRI study.

BACKGROUND Accelerated transcranial magnetic stimulation (aTMS) is an effective treatment for depression, but its neurobiological mechanisms of action remain to be elucidated. This study used high-resolution 7-Tesla MRI to investigate changes in resting-state functional connectivity (rs-FC) of the subgenual anterior ci...

Microstructural changes in prefrontal-cingulate networks following bilateral dorsomedial prefrontal cortex-targeted intermittent theta-burst stimulation in treatment-resistant depression: A diffusion tensor imaging study.

Intermittent theta-burst stimulation (iTBS) is a transcranial magnetic stimulation protocol used in treatment-resistant depression (TRD), yet its effects on brain structural connectivity remain incompletely understood. The dorsomedial prefrontal cortex (dmPFC), a key node within emotion regulation networks, has emerged...
