TL;DR

Simultaneous OPLL and OLF in the same thoracic segment is very challenging and risky to treat, regardless of the surgical method selected.

Abstract

Thoracic ossification of the posterior longitudinal ligament (OPLL) combined with ossification of the ligamentum flavum (OLF) is an uncommon cause of myelopathy, especially in our population. The optimal treatment for this thoracic myelopathy remains controversial, and the high risk of postoperative paralysis remains a major complication. We report the case of a 66-year-old man with progressive spinal cord compression due to simultaneous OPLL and OLF at T2–T3. Posterior decompression by T2–T3 laminectomy was performed without extirpating the OPLL. Postoperatively, symptoms improved greatly, with residual hyperreflexia and grade 3/5 muscle strength in the lower extremities. Conclusion: Simultaneous OPLL and OLF in the same thoracic segment is very challenging and risky to treat, regardless of the surgical method selected. Early surgery should be considered for OPLL and OLF in the thoracic spine.

Posterior circumferential decompression for multilevel thoracic ossification of the posterior longitudinal ligament: a case report and literature review

Multilevel thoracic ossification of the posterior longitudinal ligament (T-OPLL), especially when it involves more than five consecutive vertebrae, remains surgically challenging. In recent years, a strategy focused on “spinal cord de-tension” has been proposed for such extensive involvements. This report describes a 5...

Full-endoscopic decompression for thoracic ossification of the ligamentum flavum in professional pitchers: surgical technique, outcomes, and return to sport. Illustrative cases

It is suggested that full-endoscopic decompression, by preserving the posterior tension band and paraspinal musculature, may represent a viable primary surgical option for T-OLF in high-demand athletes requiring preservation of spinal mobility and kinetic chain function.

Clinical Outcomes and Feasibility of Unilateral Biportal Endoscopic Decompression for Thoracic Ossification of the Ligamentum Flavum: A Retrospective Single-Center Study

Objective: Thoracic ossification of the ligamentum flavum (T-OLF) is a primary cause of thoracic myelopathy in East Asian populations. Unilateral biportal endoscopic (UBE) decompression offers a minimally invasive alternative. This study evaluates the feasibility and clinical outcomes of UBE decompression in patients w...
