The findings provide important data on pre-existing anti-AAV immunity—a key factor considered in clinical trials and gene therapy development and points to the potential use of new sources of capsids with potentially distinct biological properties, which could contribute to the advancement of gene therapies.

Abstract

Vectors based on adeno-associated viruses (AAVs) are widely used as a delivery system in gene therapy. However, preexisting immunity against AAV remains a major limitation. This study examines the prevalence of AAV genomes and anti-AAV2 immunity in a Polish cohort of adult patients undergoing planned surgical interventions, which serves as a random sample of the general Polish population. The presence of AAV capsid sequences was analyzed in tissue samples using PCR and endogenous AAVs were detected in 33.95% of samples. Anti-AAV2 IgG antibodies were measured by a single point ELISA and were present in 80.93% of patients. Neutralizing activity was evaluated using an AAV2 transduction inhibition assay in HEK293T cells, revealing a 60% reduction in functional activity by patients’ sera. Tissue AAV PCR-positivity was associated with stronger neutralizing activity although the correlation with seropositivity was not as strong. These results suggest that pre-existing humoral immunity against AAV is common in the Polish adult population, indicating frequent exposure to naturally occurring AAVs. The findings provide important data on pre-existing anti-AAV immunity—a key factor considered in clinical trials and gene therapy development. Furthermore, the identification of naturally occurring AAV variants in human tissues points to the potential use of new sources of capsids with potentially distinct biological properties, which could contribute to the advancement of gene therapies.

Prevalence of adeno-associated virus type 3B neutralizing antibodies in cohorts of healthy donors and Wilson's disease adults.

Adeno-associated virus (AAV) is widely used for liver-directed gene therapy, with AAV serotype 3B (AAV3B) showing particularly high tropism for human hepatocytes. However, pre-existing neutralizing antibodies (NAbs) against AAV represent a major barrier to efficient gene transfer and patient eligibility. We have recent...

Global seroprevalence of neutralizing and total antibodies against AAV vectors and their impact on the clinical gene therapy landscape: a scoping review

A scoping review of 50 studies published between 1968 and 2026 found AAV2, AAV8, and AAV5 were the most extensively studied serotypes and seroprevalence increased consistently with age and varied by geographic region, although the limited number of studies from Africa and Latin America precludes robust regional conclus...

Complement Activation Within Minutes Following Systemic Administration of AAV in the Presence of AAV Immunity.

A challenge of adeno-associated virus (AAV)-mediated gene therapy is the high prevalence of preexisting anticapsid antibodies, which can reduce vector efficacy and, after high-dose systemic administration, promote formation of antibody-vector complexes that activate complement. Complement-mediated toxicities reported a...

B-216 Validation and Optimization of AAV9 Total Antibody Testing in Gene Therapy

Adeno-associated virus serotype 9 (AAV9) is widely used as a gene therapy vector. Pre-existing anti-AAV9 antibodies can reduce transduction efficiency and pose safety risks, making accurate antibody detection essential for patient selection and immune monitoring. However, despite its importance, current laborator...

IgM-and IgG-cleaving enzyme facilitates AAV gene transfer in AAV-seropositive macaques.

AAV gene therapy is a promising approach for long-term delivery of antiviral proteins that can control HIV replication, but pre-existing circulating AAV-specific antibodies can preclude successful AAV transduction and pose a challenge for AAV vector re-administration in cases where transgene protein expression falls be...
