Abstract

Background: Endoscopic Retrograde Cholangiopancreatography (ERCP) Has Largely Supplanted Open Surgical Exploration for Biliary and Pancreatic Ductal Disease, Yet Outcome Data from Tertiary Centre’s in Bangladesh. Objective: This Study Evaluated the Procedural Success Rate, Complication Profile, And Clinical Indication of ERCP Performed at A Single Tertiary Surgical Department in Northern Bangladesh. Methods: A Retrospective Case Series Of 60 Consecutive Patients Undergoing ERCP Between July 2025 and July 2026 Was Conducted. Procedures Were Performed by Experienced Endoscopists Under Fluoroscopic Guidance. Success Was Defined as Complete Ductal Clearence or Adequate Drainage with Symptom Resolution. Data Were Analyzed Descriptively Using SPSS V26.0 Results: Of 60 Patients ,45 (75%) Achieved Procedural Success. Right Upper Abdominal Pain (43.72%) And Jaundice (34,58%) Were the Leading Presentations. Stone Extractions With Sphincterotomy was The Dominant Intervention (32,71%) Followed By Biliary Stenting (8,18%) And Sphincterotomy Alone (5,11%) Failed Cannulation Accounted For 46%(7/15) Of Unsuccessful Procedures, Followed By Difficult Anatomy (20%),Inaccessibility (13%)And Bleeding (13%).Conclusion: ERCP  Achieved A (75%) Procedural Success Rate In This Cohort, Consistent With Reported Ranges For Comparable Tertiary Settings, Failed Cannulation Remains The Principle Barrier To Success, Underscoring The Value Of Endoscopist Experience And Structured Escalation Protocols.
