This review critically evaluates evidence from interventional studies demonstrating the impact of pharmacist-led services, including medication therapy management (MTM), patient counselling, lifestyle modification support, and continuous monitoring, and the integration of emerging technologies such as mobile health, telepharmacy, clinical decision support systems, and artificial intelligence (AI) in optimising pharmaceutical care delivery.
Abstract
Type II Diabetes Mellitus (T2DM) is a rapidly growing global health concern associated with significant morbidity, mortality, and economic burden. Despite advances in pharmacotherapy, achieving optimal glycemic control remains a major challenge due to poor adherence, complex treatment regimens, and fragmented healthcare delivery. Pharmaceutical care, a patient- centred approach led by pharmacists, has emerged as an effective strategy to improve diabetes management outcomes. This review critically evaluates evidence from interventional studies demonstrating the impact of pharmacist-led services, including medication therapy management (MTM), patient counselling, lifestyle modification support, and continuous monitoring. These interventions have been consistently associated with significant reductions in HbA1c (0.5–2%), improved medication adherence, enhanced quality of life, and decreased hospitalisation rates. The review further explores the integration of emerging technologies such as mobile health (mHealth), telepharmacy, clinical decision support systems (CDSS), and artificial intelligence (AI) in optimising pharmaceutical care delivery. Key barriers, including healthcare system limitations, lack of reimbursement policies, and interdisciplinary coordination challenges, are also discussed.
Background: Type 2 diabetes mellitus (T2DM) is a chronic metabolic disease with a continuously increasing global prevalence. Successful T2DM management largely depends on patients' adherence to pharmacological therapy and overall disease management. However, medication adherence remains a major challenge, often resulti...

Background: Complex treatment regimens in hemodialysis patients often lead to drug-related problems (DRPs), non-adherence, diminished quality of life, and a high medical cost burden. This presents an opportunity to implement pharmaceutical care to optimize patients' therapeutic outcomes. Objective: This study aimed to...

Background: Type 2 Diabetes Mellitus (T2DM) represents a major global healthcare challenge requiring continuous, lifelong multi-factorial self-management. While mobile health (mHealth) technologies are increasingly deployed across clinical pathways to enhance glycemic control, their long-term therapeutic effectiveness...
