TL;DR

Findings support this approach for appropriately selected patients in routine clinical care for intermediate- and high-risk prostate cancer, as contemporary real-world evidence remains limited.

Abstract

Purpose

To evaluate the safety and early oncologic outcomes of external beam radiation therapy (EBRT) combined with a low-dose-rate (LDR) brachytherapy boost in patients with intermediate- and high-risk prostate cancer, as contemporary real-world evidence remains limited.

Methods

AND MATERIALS Investigators retrospectively reviewed 307 patients treated at a large tertiary cancer center between 1999 and 2024. Median age was 66 years; 36% had intermediate-risk and 64% had high- or very high-risk disease, and 91% received androgen deprivation therapy. Late gastrointestinal (GI) and genitourinary (GU) toxicities were defined as occurring more than three months after radiation initiation and graded using CTCAE version 5. Biochemical failure was defined using Phoenix criteria. Outcomes were summarized descriptively; Cox regression analyzed overall survival (OS) and biochemical recurrence-free survival (bRFS), while Fine-Gray models evaluated late toxicity with death as a competing risk.

Results

At a median follow-up of 4.6 years, 22 patients developed biochemical recurrence. Five-year bRFS was 98% for intermediate-risk disease and 90% for high- or very high-risk disease; OS and bRFS were 92% and 95%, respectively. Grade 3 or higher GU and GI toxicity occurred in 3.2%, and no significant predictors of severe late toxicity were identified.

Conclusion

These findings support this approach for appropriately selected patients in routine clinical care.

High-dose-rate brachytherapy boost with external beam radiotherapy for localized prostate cancer: Long-term outcomes and toxicity in 607 predominantly high-risk patients.

OBJECTIVE To report long-term outcomes and late toxicity after external beam radiotherapy (EBRT) with single-fraction high-dose-rate brachytherapy boost (HDR-BTB) for localized prostate cancer, examining HDR-BT sequencing and risk stratification. METHODS AND MATERIALS We retrospectively analyzed 607 (2012-2023) men t...

Dose-Escalated Focal Salvage High-Dose-Rate Brachytherapy and Genomic Classifier Risk Stratification in Radiorecurrent Prostate Cancer: The F-SHARP Phase I/II Clinical Trial.

PURPOSE Prospective data on the management of intraprostatic radiorecurrence (IPR) after definitive radiotherapy (RT) and use of genomic classifiers (GC) in this setting are limited. We hypothesized that dose-escalated focal salvage high-dose-rate brachytherapy (DE-fsHDR) would be safe and effective, and that GC would...

Image-guided high-dose-rate brachytherapy after short-course external beam radiotherapy for elderly/frail rectal cancer patients.

PURPOSE For elderly and frail patients with rectal cancer, unfit for radical surgery or extensive chemoradiation schedules, radiotherapy dose escalation by use of high-dose-rate brachytherapy (HDR-BT) may be an alternative organ-preserving approach. METHODS 15 elderly/frail patients (median age, 83 years; ECOG 1‑2: 8...

Long-term outcomes of high-dose hyperfractionated thoracic radiotherapy for limited-stage small-cell lung cancer.

Concurrent chemoradiotherapy with thoracic radiotherapy of 45 Gy in 30 twice-daily fractions is the standard treatment for limited-stage small-cell lung cancer (LS-SCLC). However, the optimal thoracic radiotherapy dose remains uncertain. This study aimed to evaluate the efficacy and safety of high-dose hyperfractionate...
