TL;DR

This cohort illustrates the broad clinical spectrum of STAT1 GOF disease, including infectious, inflammatory, autoimmune, and hematological manifestations, and BCG disease and tuberculosis were observed in a subset of patients, highlighting the complexity of clinical presentation and the importance of comprehensive evaluation and follow-up.

Abstract

Background Germline Signal Transducer and Activator of Transcription 1 (STAT1) gain-of-function (GOF) variants enhance cellular responses to IL-27 and type I, II, and III interferons, predisposing to fungal, bacterial, and viral infections and immune dysregulation. Methods Mexican patients with pathogenic STAT1-GOF variants identified at referral centers were enrolled. Results Thirty patients from 17 kindreds were included. Fifteen variants were identified, including the novel C324W variant, confirmed as GOF by functional studies. All patients developed mucocutaneous candidiasis. Among 25 BCG-vaccinated patients, 24% developed BCG disease. According to the Graham criteria, seven patients (23%) with tuberculosis were classified as confirmed (n=1), possible (n=3), and probable (n=3) cases. Three patients developed both BCG disease and tuberculosis. Autoinflammatory and autoimmune manifestations occurred in 50% and 40%, respectively. Two patients developed infection-triggered macrophage activation syndrome. Seven patients (23%) died. Conclusions This cohort illustrates the broad clinical spectrum of STAT1 GOF disease, including infectious, inflammatory, autoimmune, and hematological manifestations. BCG disease and tuberculosis were observed in a subset of patients, highlighting the complexity of clinical presentation and the importance of comprehensive evaluation and follow-up.

The broad spectrum of immune dysregulation: clinical cases

INTRODUCTION: Inborn errors of immunity are classified according to the many possible genetic defects, and 25% manifest as immune dysregulation. CTLA-4 (cytotoxic T-lymphocyte antigen 4) haploinsufficiency has been associated with variable clinical presentations, ranging from asymptomatic carriers (30%) to patients wit...

Mild Phenotype in Functionally Validated STAT1 Gain‐of‐Function Disease Due to a Novel DNA‐Binding Variant

Signal transducer and activator of transcription 1 (STAT1) gain‐of‐function (GOF) disease is associated with chronic mucocutaneous candidiasis, infections, autoimmunity, and immune dysregulation; however, mutation‐level determinants of phenotypic variability remain incompletely defined. We aimed to describe the clinica...

Genetic Association of IFITM3 rs12252 Variant with Severe COVID-19 Outcomes in Syrian Patients.

INTRODUCTION Following the COVID-19 pandemic, increasing attention has been directed toward host genetic factors that influence disease severity. The interferon-induced transmembrane protein 3 (IFITM3) plays a key role in restricting viral entry, and its rs12252 polymorphism has been linked to impaired antiviral defens...

Biologics and JAK inhibitors for atopic manifestations in inborn errors of immunity: a systematic review

Background Atopy might be the initial or dominant manifestation in patients with inborn errors of immunity (IEIs). Biologics and Janus kinase (JAK) inhibitors have emerged as targeted options for atopic disorders in IEIs refractory to conventional treatment. Methods We conducted a systematic review of PubMed, Web of Sc...

The prevalence and impacts of NUDT15 variants in pediatric-onset inflammatory bowel disease patients in Taiwan.

BACKGROUND Thiopurines are important therapeutic agents for patients with inflammatory bowel disease (IBD), and the Nudix hydrolase 15 (NUDT15) gene has been reported to be associated with the safety of thiopurine use. We investigated the prevalence of NUDT15 variants and their impacts in the pediatric-onset IBD group...

Dominant lung involvement and heterozygous IFIH1 variants.

BACKGROUND Heterozygous gain-of-function (GoF) variants in IFIH1 are associated with rare autoinflammatory diseases characterized by heightened interferons-stimulated gene (ISG) signatures. These include Aicardi-Goutières syndrome, presenting with slowly progressive spastic paraparesis of the lower limbs, intracerebral...
