Structural innovation and therapeutic potential of oxazolidinone antibacterials

Oct 2026 · Journal of Applied Pharmaceutical Research · 0 citations

Abstract

Background: Oxazolidinones mark a major advance in antibacterial therapy, being the first new antibiotic class introduced into hospitals in over three decades. Their relevance is underscored by the global crisis of antimicrobial resistance (AMR), particularly against multidrug‑resistant Gram‑positive pathogens. Methodology: We conducted a systematic literature review using ScienceDirect and Google Scholar. We selected studies based on mechanistic insights, clinical efficacy, pharmacokinetics, safety, and resistance pathways. We emphasized FDA-approved agents (linezolid and tedizolid) and next-generation derivatives in clinical development. Results and Discussion: Linezolid and tedizolid inhibit bacterial protein synthesis by binding to the 23S rRNA of the ribosomal initiation complex. Tedizolid is more potent, has reduced mitochondrial toxicity, and inhibits monoamine oxidase less than linezolid. Resistance mechanisms include ribosomal mutations, methylation, efflux pumps, and enzymatic modifications. Structural innovations in tedizolid—such as its hydroxymethyl substituent—enhance ribosomal binding and mitigate cfr‑mediated resistance. The pipeline includes promising agents like sutezolid, delpazolid, contezolid, radezolid, and TBI‑223, many designed to overcome toxicity and resistance limitations. Comparative analysis highlights tedizolid’s improved therapeutic index and the convenience of once-daily dosing. Next‑generation oxazolidinones expand the scope to tuberculosis and other resistant infections, reflecting rational drug design informed by mechanistic understanding. Challenges remain in mitigating resistance and ensuring long‑term safety. Conclusion: Oxazolidinones remain vital in combating AMR. Continued innovation, structural refinement, and global stewardship are essential to sustain their efficacy and extend the clinical lifespan of this antibiotic class.
