Highlights • Inspiratory muscle strength measurement is safe in liver transplant ICU patients.• No adverse events were observed during inspiratory strength assessment.• Lower inspiratory strength is associated with worse postoperative outcomes.• Inspiratory muscle weakness may identify high-risk patients early.
