MRI-Based Retrospective Analysis of Knee Joint Injuries: Patterns and Prevalence

Abstract

Background: Knee injuries are common musculoskeletal problems affecting stability, weight-bearing, and movement. Trauma, sports activities, overuse, and degenerative changes contribute to internal derangements, and accurate diagnosis is essential to prevent chronic pain, joint instability, and osteoarthritis. MRI is the preferred imaging modality due to its ability to visualize both soft tissue and bony structures in multiple planes. Objective: To retrospectively analyze MRI findings in patients with knee injuries and determine the frequency, pattern, and associated abnormalities. Methods: A ‘ retrospective observational study was conducted on 120 patients who underwent MRI for knee injuries at Sonoscan Pvt.Ltd, Malda, West Bengal, India. Data on age, gender, clinical history, side of injury, and MRI findings were collected using a structured data sheet. Standard MRI protocols including T1, T2, Proton Density, and fat-suppressed sequences in sagittal, coronal, and axial planes were used. Descriptive statistics were applied to assess the frequency and distribution of injuries. Results: Most patients were males (65%), and the 21–30 years age group was most affected (30%). Right knee injuries were slightly more frequent (56.7%). ACL tears (38.3%) and medial meniscus tears (33.3%) were the most common injuries. Joint effusion (43.3%) and bone marrow edema (28.3%) were frequently associated findings, while cartilage injuries were observed in 16.7% of cases. Conclusion: MRI provides a detailed evaluation of knee injuries, aiding diagnosis and management. ACL and medial meniscus injuries are predominant in young adults, and MRI detection of associated abnormalities helps guide treatment and improve patient outcomes.

The Role of Magnetic Resonance Imaging in Sports Injuries: Current Applications and Clinical Importance - A Narrative Review

Background. Magnetic resonance imaging (MRI) is the reference imaging modality for evaluating sports-related musculoskeletal injuries because of its excellent soft tissue contrast and ability to assess ligaments, tendons, muscles, cartilage, and bone marrow without ionizing radiation. Aim. To summarize the clinical applications of MRI in the diagnosis and management of sports-related musculoskeletal injuries and discuss emerging developments in quantitative MRI and artificial intelligence (AI). Material and methods. A narrative review of recent reviews, systematic reviews, meta-analyses, and original studies on MRI in sports medicine was performed. Results. Conventional MRI remains essential for diagnosing anterior cruciate ligament tears, meniscal injuries, muscle injuries, Achilles tendon disorders, and bone stress injuries, supporting treatment planning and rehabilitation. Quantitative MRI enables assessment of tissue composition and microstructural changes beyond conventional imaging, while AI shows promise in image acquisition, reconstruction, automated analysis, and clinical decision support. However, further validation and protocol standardization are required. Conclusions. MRI remains central to sports injury evaluation. Advances in quantitative MRI and AI may improve diagnostic accuracy, workflow efficiency, and individualized patient management, although further prospective multicentre studies are needed before routine clinical implementation.

Timing Of MRI in Acute Lateral Ankle Sprain: Impact On Diagnostic Findings And Clinical Interpretation – A Retrospective Observational Cohort Study

Beyond the ligament: Role of bone bruising patterns and their clinical impact in acute ACL injuries

Background: Bone marrow edema (BME) is one of the common magnetic resonance imaging (MRI) findings in anterior cruciate ligament injuries. The prevalence and clinical relevance of BME vary depending on injury mechanism and timing of imaging. This study aimed to evaluate the prevalence and patterns of BME in patients with ACL tears and to correlate these patterns with mechanism of injury. Methods: In this hospital based observational study, a total of 136 patients with MRI confirmed ACL injuries were included. MRI of the affected knee was performed using standardized hospital protocols. BME patterns were categorized into pivot-shift, hyperextension, varus or valgus stress and mixed patterns. MRI findings were correlated with patient reported mechanism of injury. Results: Bone marrow edema was detected in 63 (46.3%) patients. Among 63 patients with bone marrow edema, pivot-shift pattern of bone marrow edema was the most frequently observed MRI findings seen in 29 (46.0%) patients, followed by hyperextension pattern identified in 14 (22.2%) patients. Pivot-shift pattern was predominantly associated with sports injuries seen in 21 (29.2%) patients, while hyperextension were more commonly observed followed by road traffic accidents seen in 7 (21.9%) patients. Varus, valgus and mixed patterns were frequently seen in fall and twisting injuries. A statistically significant association was observed between bone marrow edema ptterns and reported mechanism of injury (x2 = 28.4, p = 0.003). Conclusion: Systematic BME pattern analysis into routine MRI reporting may enhance diagnostic accuracy, improve clinical decision-making, and contribute to more individualized patient management strategies following ACL injury.
