Music assisted nitrous oxide oxygen inhalation sedation in pediatric dentistry: a prospective three-arm randomized clinical trial

Abstract

Nitrous oxide–oxygen inhalation sedation (NOIS) is the recommended first line pharmacological approach for managing dental anxiety in children. Music, owing to its psychoacoustic qualities and autonomic nervous system-influencing potential, emerges as a potential non-pharmacological agent in the management of dental anxiety. The aim of this study was to assess the efficacy of Indian instrumental music as an adjunct to nitrous oxide-oxygen Inhalation sedation. This prospective, three arm parallel- group randomized clinical trial was conducted at the Department of Pediatric and Preventive Dentistry, Yenepoya Dental College, Mangalore (CTRI/2024/04/065857). A total of 39 anxious child participants between the ages of 9 and 12 years (Mean age = 10.56 ± 1.14 years) who required restorative procedures under NOIS, as assessed by MCDASf were included in the study. They were equally distributed among the groups, i.e., Group A (Control Group), Group B (NOIS music through ambient speakers), and Group C (NOIS with music through earphones), each consisting of 13 participants. The intervention used was ‘Minuet in D major' from the album ‘Classicool' performed by sitar maestro Purbayan Chatterjee. Physiological responses such as systolic BP, diastolic BP, and pulse rate PR) were recorded. The earphone group (Group C) showed statistically significant change over time for all physiological parameters. p = 0.003, p = 0.011, p = 0.030, respectively, for systolic bp, diastolic bp, and PR. Intergroup comparisons revealed no statistically significant differences at baseline or post-intervention. A statistically significant between-group difference was noted during the intervention phase exclusively for PR (p = 0.014). The music intervention demonstrated high overall acceptability; 88.5% of participants reported liking the music and 84.6% indicated a desire for its continuation in future appointments (p = 1.000). The use of Indian instrumental music as an adjunct to NOIS in Earphone-delivered music demonstrated statistically significant within-group reductions in all physiological parameters, with a significant intergroup difference in pulse rate during intervention (p = 0.014). Most intergroup differences were non-significant, and these findings should be interpreted as clinically promising trends warranting larger randomized trials rather than definitive evidence of efficacy. The efficacy of music as an adjunct analgesic in reducing pain and anxiety during ultrasound-guided manual vacuum aspiration (USG-MVA): a randomized controlled trial.

BACKGROUND Ultrasound-guided manual vacuum aspiration (USG-MVA) is one of the surgical treatment options for managing early pregnancy loss. When compared to the traditional surgical method, USG-MVA is safer, more cost-effective, due to its shorter hospitalisation period andeliminates the risks associated with general anaesthesia. The objective of this study is to evaluate music therapy as an adjunct for pain and anxiety reduction during USG-MVA and assess its impact on physiological stress and patient satisfaction. METHODS This was a single-centre randomized controlled trial in a university-affiliated tertiary hospital in Hong Kong. This study aimed to randomize 176 women with first-trimester miscarriage requiring USG-MVA performed. Participants were randomly allocated to the music intervention group or control group receiving standard care in a 1:1 ratio. Pain and anxiety levels as well as patient satisfaction were assessed using physiological and psychological assessments. Primary outcome is pain reduction, secondary outcomes include anxiety, physiological stress and patient satisfaction. RESULTS Music reduced severe pain (Visual Analogue Scale > 70) during USG-MVA (27.3% vs. 47.7%, P < 0.05) but showed no significant effects on anxiety, stress biomarkers, or satisfaction (all P > 0.05). CONCLUSIONS Music therapy effectively mitigates procedural pain during USG-MVA but does not significantly influence anxiety or satisfaction. Its low-cost, non-invasive nature supports clinical utility.

Evaluation of the Effects of Computer-Controlled and Conventional Local Anesthesia on Pain and Anxiety in Children With Molar Incisor Hypomineralization: A Split-Mouth Study.

BACKGROUND Computer-controlled anesthesia delivery systems (CCLADs), such as SleeperOne, are available as an alternative to traditional anesthesia for the treatment of teeth with molar incisor hypomineralization (MIH). AIM This study aimed to evaluate the effects of the SleeperOne and the conventional local anesthesia technique on pain and anxiety in children with MIH. DESIGN Twenty-eight children aged 6-12 were included in the study. Patients were divided into 2 groups (n = 28): Conventional anesthesia group and SleeperOne group. The Children's Fear Survey Schedule-Dental Subscale (CFSS-DS) was administered to all participants prior to treatment. Pain and anxiety levels were assessed using both objective measures (pulse rate, oxygen saturation [SpO2], and salivary cortisol levels) and subjective scales (Wong-Baker Pain Rating Scale [WBS], Visual Analog Scale [VAS], and Face, Legs, Activity, Crying, Consolability Scale [FLACC]). RESULTS Cortisol levels were significantly higher in the conventional anesthesia group (p < 0.001). In the after treatment values, mandibular cortisol levels in participants without hypersensitivity and total cortisol levels in participants with hypersensitivity were significantly lower in the SleeperOne anesthesia group (p = 0.009 and p = 0.017). CONCLUSIONS Lower salivary cortisol levels, a physiological marker of stress, were observed in the SleeperOne group during the treatment of MIH-affected teeth.
