Abstract

Proactive health behavior may enhance maternal well-being during pregnancy, yet the concept remains insufficiently defined in pregnancy-related care and nursing practice. This study used Rodgers' evolutionary method of concept analysis to clarify the concept and its relevance for holistic nursing practice. A structured search of 7 electronic databases identified 17 eligible publications from 2015 to 2026. The analysis identified 3 antecedent domains: individual characteristics, interpersonal resources, and sociodemographic and cultural contexts. Six core attributes were delineated: proactive cognitive engagement, behavioral confidence, accessibility of health information, psychological balance, adaptive behavior regulation, and social support-related benefits. The concept was further organized into 2 interrelated phases: initiation and maintenance. Proactive health behavior during pregnancy is thus understood as a dynamic, phase-based process with both empowering and potentially burdensome dimensions, offering a conceptual foundation for future research and holistic nursing support.
