TL;DR

A systematic review and exploratory meta-analysis of studies evaluating perfusion or hemodynamic imaging in adults with low-NIHSS LVO stroke found large Tmax-defined hypoperfusion volumes, larger perfusion lesion volumes, and impaired hemodynamic reserve were associated with early neurological deterioration or poor functional outcome.

Post-Endovascular Thrombectomy Perfusion Imaging (CT and MR) as a Prognostic Biomarker and Patient-Selection Tool for Adjuvant Therapy: A Systematic Review, Meta-Analysis, and Clinical Threshold Framework.

BACKGROUND AND PURPOSE Despite high rates of macrovascular recanalization, approximately half of patients with large vessel occlusion stroke fail to achieve functional independence after endovascular thrombectomy (EVT). Residual tissue-level perfusion abnormalities - detectable on post-procedural CT perfusion (CTP) - h...

Endovascular Thrombectomy for Acute Ischemic Stroke Due to Large Vessel Occlusion with Large Ischemic Core: A Systematic Review and Meta-Analysis

Background: Multiple trials have evaluated the benefits of endovascular thrombectomy (EVT) in patients with large-core ischemic stroke. This study aimed to determine the efficacy and safety of EVT compared with medical management (MM) alone in patients with large ischemic core infarcts. Methods: This study was register...

Efficacy and safety of magnetic resonance imaging- vs. computed tomography-guided reperfusion in acute ischemic stroke: a systematic review and meta-analysis

Objective Acute ischemic stroke (AIS) is a leading cause of death and disability. Timely reperfusion improves outcomes, but optimal patient selection depends on neuroimaging; CT is fast and widely available, whereas MRI offers greater precision in identifying salvageable brain tissue. This systematic review (SR) and me...

Endovascular treatment and clinical outcomes in minor ischemic stroke due to large-vessel disease: a retrospective study

Background Patients with minor ischemic stroke may still experience early neurological deterioration when severe intracranial stenosis or occlusion is present. The optimal role of endovascular treatment (EVT) in this imaging-defined population remains uncertain. This study aimed to evaluate clinical outcomes associated...

Impact of balloon guide catheter use on procedural efficacy and early neurological outcomes in endovascular thrombectomy for anterior circulation acute ischemic stroke: a propensity-score-matched analysis.

BACKGROUND The role of balloon guide catheters (BGC) in endovascular thrombectomy for acute ischemic stroke remains controversial, with conflicting evidence regarding clinical outcomes. We evaluated the impact of BGC use on periprocedural metrics and early neurological improvement (ENI). METHODS In this retrospective...

Retrospective cohort analysis of early post treatment follow-up MRI biomarkers of collateral status associated with long-term outcomes in large vessel occlusion acute ischemic stroke.

NFS on post-treatment MRI is a robust imaging biomarker of persistent perfusion deficits and associated with decreased likelihood of successful long-term outcomes even after successful reperfusion.
