TL;DR

Altered right precuneus spontaneous activity was associated with perceived stress-related cognitive appraisal, particularly stress-coping self-efficacy, in FEDN patients with MDD after controlling for depressive and anxiety symptom severity.

Abstract

Objective

First-episode, drug-naïve (FEDN) patients with major depressive disorder (MDD) frequently exhibit disturbances in stress perception and cognitive function, yet the neural correlates remain unclear. This study investigated associations of altered regional homogeneity (ReHo) in the right precuneus with subjective stress perception in FEDN MDD.

Methods

We enrolled 139 FEDN patients and 65 healthy controls. Propensity score matching (PSM) balanced age, sex, and head motion, yielding 63 pairs. Resting-state fMRI was acquired and preprocessed; whole-brain ReHo maps were computed. Voxel-wise group comparison with GRF correction identified decreased ReHo in the right precuneus, which was defined as the region of interest. Within patients, partial correlations between right precuneus ReHo and Perceived Stress Scale (PSS) total, helplessness, and self-efficacy scores were examined, controlling for age, sex, mean framewise displacement, HAMD-17, and HAMA.

Results

Compared to controls, FEDN patients showed significantly decreased right precuneus ReHo. In patients, after controlling for age, sex, mean framewise displacement, HAMD-17 score, and HAMA score, right precuneus ReHo was negatively associated with the self-efficacy dimension of the PSS (r = -0.497, p = 0.011). A nominal positive association was also observed between right precuneus ReHo and the PSS total score (r = 0.399, p = 0.048), whereas no significant association was found with helplessness.

Conclusion

Altered right precuneus spontaneous activity was associated with perceived stress-related cognitive appraisal, particularly stress-coping self-efficacy, in FEDN patients with MDD after controlling for depressive and anxiety symptom severity. These findings highlight a potential role of the right precuneus in stress-coping self-efficacy in FEDN MDD patients.

Distinct and shared structural covariance reductions of the habenula in first-episode drug-naive and recurrent major depressive disorder.

Emerging evidence suggests that habenula (Hb) dysfunction plays an important role in major depressive disorder (MDD), with prior functional and structural studies implicating Hb-related networks in emotional and reward processing. However, it remains unclear whether structural covariance of Hb differs between first-epi...

EEG Microstate Alterations in Anhedonia Subtypes of Major Depressive Disorder and Their Modulation by Intermittent Theta‐Burst Stimulation

Correlation analyses revealed that, in the HA group, greater anhedonia improvement was associated with increased MS_A duration and decreased MS_B duration, suggesting that microstate changes may serve as a treatment response marker.

Multilevel dynamic abnormalities of suicidal ideation in major depressive disorder from regional dynamics to network states.

A multi-level perspective on the dynamic neural mechanisms of suicidality is provided and regional and network-level dynamic abnormalities may differentially contribute to trait SI, recent SI, and treatment-related improvements.

Greater task-independent overall beta power differentiates depressed patients from healthy controls during an emotion word task.

BACKGROUND The identification of depression based on electroencephalographic (EEG) measures may be at least as accurate as commonly used screening tests in clinical practice, however, less is known about how to distinguish between trait-level, endogenous and state-level, reactive subtypes of depression on a neurophysio...

Analysis of brain volumetry and functional connectivity during resting state in patients with borderline personality disorder.

BACKGROUND Borderline Personality Disorder (BPD) involves affective instability, impaired emotion regulation, and self-injurious behavior, often linked to childhood adversity. Although prefrontal and insular abnormalities have been reported, the relationship between structural and functional changes in BPD is not well...
