TL;DR

A five-component hybrid framework is proposed: phase-gate architecture, dose-allocation logic, caregiver-integration tiers, a remote-capable outcomes battery, and safety guardrails, which applies to stroke, polytrauma, progressive neurological disease, and geriatric falls populations.

Abstract

Hybrid rehabilitation models combining in-clinic and home-based therapy address resource constraints and the heterogeneous needs of patients with complex neurological conditions. Each arm has independent support: telerehabilitation, early supported discharge, and caregiver-mediated exercise carry Cochrane-level evidence, while in-clinic intensity and task-specific training remain standard for motor recovery. No integrative framework, however, prescribes how dose, content, timing, and outcome measurement should be allocated across settings when stroke, traumatic brain injury, progressive disease, and fall risk overlap in a single patient. This paper synthesises fifteen high-quality references and proposes a five-component hybrid framework: phase-gate architecture, dose-allocation logic, caregiver-integration tiers, a remote-capable outcomes battery, and safety guardrails. Application to stroke, polytrauma, progressive neurological disease, and geriatric falls populations is mapped. Implications for civilian home-health physical therapy practice and cost-effectiveness modelling are discussed.

Technology-Assisted Rehabilitation Gym for at-home Training (TARGeT) for stroke survivors: A study protocol for evaluating feasibility, safety, usability, clinical benefit, and costs

Stroke is a leading cause of long-term disability, with more than half of survivors experiencing persistent upper limb impairments. Despite clear evidence that high-dose, repetitive, task-specific training is essential for motor recovery, many stroke survivors do not receive a sufficiently high therapy dose – particula...

Beyond the Diagnosis: An Integrated Perspective on Strategic Dosing in Neurologic Motor Rehabilitation. A Scoping Review and Collaborative Viewpoint

Background Current neurorehabilitation often relies on static, diagnosis-driven protocols or generic exercise administration, contributing to suboptimal long-term outcomes. There is a clinical need for a cohesive, mechanism-based framework to guide therapeutic reasoning, dosing, and longitudinal adaptation. Methods A s...

Acute Care Group-Based Physical Therapy on an Inpatient Geriatrics Unit: Preliminary Data.

BACKGROUND Hospitalized older adults are at high risk for hospital-associated disability, but one-to-one physical therapy (PT) models often cannot meet demand for skilled mobility progression. Group-based PT is effective in outpatient and rehabilitation settings, but its feasibility in acute inpatient care is unknown....

Augmenting Parkinson’s Rehabilitation at Home

This thesis focused on the purposeful co-development and early implementation of home-based exergaming therapy with Strolll Rehab, alternated with in-clinic therapy. The therapy targets gait, balance, walking adaptability, and ultimately fall risk. Home-based therapy was asynchronously prescribed and managed by trained...

Analysis of Home-Based Rehabilitation Robotics: Lessons Learned from Nationwide Direct-to-Consumer Deployment.

INTRODUCTION Millions of stroke survivors in the US have sensory-motor impairments that significantly impact quality of life and place a long-term burden on families, communities, and the healthcare system. Neurorehabilitation aims to improve function, typically through repetitive practice of movements and task-related...
