TL;DR

Histological GBM, IDH mutation status, and EOR ≥90% were significant predictors of OS in patients with insular gliomas, and awake functional mapping for insular gliomas is a useful surgical approach, as it minimizes neurological deficits and allows preservation of neurocognitive functions.

Abstract

Objective

This study aimed to evaluate prognostic factors and oncofunctional outcomes, including neurological and neurocognitive preservation, in patients with World Health Organization 2021-classified insular lower-grade gliomas and glioblastoma who underwent awake brain mapping.

Methods

We retrospectively analyzed 36 consecutive patients with insular glioma, including LrGGs and GBM, who underwent awake surgery between December 2012 and January 2021.

Results

The group of patients with IDH-mutant LrGG included 23 cases, while those with IDH-wildtype GBM with histological features of LrGG and GBM included five and eight patients, respectively. The postoperative mean Wechsler Memory Scale-Revised scores for General Memory (Z=-0.56, p = 0.04) and Delayed Recall (Z=-0.73, p = 0.04) significantly improved compared with their preoperative values after awake surgery. The median overall survival (OS) of patients with IDH-mutant LrGGs was significantly longer (not reached) than that of patients with IDH-wildtype GBM with histological LrGG (15 months, p < 0.001) and IDH-wildtype GBM with histological GBM (12 months, p < 0.001). The multivariate models showed that OS rates were significantly related to pathology [histological GBM vs. histological LrGG, hazard ratio (HR) = 6.41, p = 0.04], IDH1 or IDH2 status (mutation vs. wildtype, HR = 0.04, p = 0.01), and final EOR (≥90% vs. <90%, HR = 0.12, p = 0.04).

Conclusions

Histological GBM, IDH mutation status, and EOR ≥90% were significant predictors of OS in patients with insular gliomas. Moreover, awake functional mapping for insular gliomas is a useful surgical approach, as it minimizes neurological deficits and allows preservation of neurocognitive functions.

Awake Craniotomy for Eloquent Region Glioblastoma Classified by Tumor Location—A Retrospective and Prospective Cohort Study

ABSTRACT Introduction The efficacy of awake craniotomy (AC) with intraoperative mapping for glioblastoma (GBM) in eloquent regions remains debated. This study aims to evaluate functional and survival outcomes of GBM patients undergoing AC stratified by tumor locations. Methods A combined retrospective (2015–2023, n = 1...

nTMS-determined cortical excitability is associated with overall survival in patients with motor-eloquent glioblastoma

Preoperative prognostication in glioblastoma remains challenging despite advances in histomolecular classification. Navigated transcranial magnetic stimulation (nTMS) enables non-invasive assessment of cortical excitability, but its role as a survival biomarker remains unclear. To determine whether preoperative nTMS-de...

Association of Early Postoperative MRI-Based RANO-Resect With Survival in WHO 2021-Defined IDH-Mutant High-Grade Gliomas

Direct evidence for postoperative residual-burden stratification remains limited in WHO 2021-defined isocitrate dehydrogenase (IDH)-mutant high-grade gliomas. We evaluated whether early postoperative MRI-based Response Assessment in Neuro-Oncology resect (RANO-resect) was associated with overall survival (OS) and...

Impact of aggressive treatment on outcomes in IDH-wildtype glioblastoma patients with leptomeningeal dissemination.

PURPOSE Prognosis of leptomeningeal dissemination (LMD) in IDH-wildtype glioblastoma (GBM) patients is dismal, but prognostic factors and optimal treatment strategy for LMD are still not clearly defined. This multi-institutional study aims to analyze clinical factors that affect the outcomes of GBM patients with LMD....
