In immunocompromised patients with multifocal bowel thickening and perforation, CMV enteritis should be considered even when preoperative imaging suggests an alternative source.
Abstract
Cytomegalovirus (CMV) infection of the gastrointestinal tract occurs predominantly in immunocompromised patients and most commonly affects the stomach and colon, while clinically significant small bowel disease and perforation are rare. We report a case of a 79-year-old man with chronic lymphocytic leukaemia (CLL), developing a superimposed CMV infection leading to hollow viscus perforation. CMV inclusions were confined to ulcerated mucosa overlying areas of neoplastic infiltration and were absent in intervening macroscopically normal bowel, supporting a synergistic clinicopathological mechanism rather than incidental co-infection. This case highlights that in immunocompromised patients with multifocal bowel thickening and perforation, CMV enteritis should be considered even when preoperative imaging suggests an alternative source.
Gastrointestinal mucormycosis is a rare but life-threatening invasive fungal infection affecting immunocompromised individuals. Emphysematous gastritis is a rare, fulminant manifestation with high mortality. We report a case of a woman in her 80s with multiple comorbidities, including diabetes mellitus and necrotizin...

Cytomegalovirus (CMV) reactivation in Mycobacterium tuberculosis (TB) infection is rare and has been only reported in immunocompromised patients. In this case report, we describe a patient with Type 2 diabetes with acute on chronic abdominal pain found to have miliary TB with terminal ileitis and ascending colon ulcers...

Protein-losing enteropathy (PLE) complicated by cytomegalovirus (CMV) enterocolitis is rare in adults. We report a fatal case of refractory PLE complicated by CMV enterocolitis after chemotherapy for multiple myeloma. A 71-year-old woman developed severe diarrhea and hypoalbuminemia after a single course of bortezomib-...

Endobronchial aspergillosis (EBA) is an uncommon manifestation of Aspergillus airway disease characterized by fungal involvement of the tracheobronchial lumen. It may present with non‐specific respiratory symptoms, including dyspnea, cough and hemoptysis, and can mimic endoluminal airway tumours. Predisposing factors i...

Persistent ileal ulcerations in Crohn's disease (CD) typically suggest refractory inflammation, yet rare fungal infections may coexist silently. We report a 22-year-old immunocompetent man with CD and seronegative spondyloarthritis who achieved clinical remission on ustekinumab but exhibited persistent ileal ulce...
