Serum Interleukin-6, Tumour Necrosis Factor-α and Lipopolysaccharide-binding Protein in Pseudomonas aeruginosa-Associated Diabetic Foot Ulcers: A Cross-sectional Study of Clinical, Microbiological and Inflammatory Profiles from Al-Najaf, Iraq

Abstract

Background: Pseudomonas aeruginosa is frequently isolated from infected diabetic foot ulcers (DFUs), but whether its recovery is associated with a distinct systemic inflammatory profile remains uncertain. This study compared serum interleukin-6 (IL-6), tumour necrosis factor-α (TNF-α) and lipopolysaccharide-binding protein (LBP) in patients with and without P. aeruginosa-associated DFUs and in healthy adults. Methods: This cross-sectional study included 120 participants in Al-Najaf, Iraq: 40 patients with clinically infected DFUs yielding P. aeruginosa, 40 with infected DFUs without P. aeruginosa and 40 healthy controls. Bacterial isolates underwent disc-diffusion susceptibility testing, and serum biomarkers were measured by ELISA. Analyses adjusted for age, sex and body-mass index. Results: Mean IL-6, TNF-α and LBP concentrations were higher in P. aeruginosa-positive than P. aeruginosa-negative DFUs (26.7 vs 16.5 pg/mL, 100.1 vs 59.1 pg/mL and 14.1 vs 10.9 µg/mL, respectively; all P < 0.001), with lower values in healthy controls. Twenty of 40 P. aeruginosa isolates were multidrug resistant, although biomarker concentrations did not differ significantly by resistance status. LBP (AUC 0.946) and TNF-α (AUC 0.941) discriminated P. aeruginosa-positive from P. aeruginosa-negative DFUs better than C-reactive protein (AUC 0.812). LBP was the only independent correlate of P. aeruginosa isolation. Conclusion: P. aeruginosa-positive DFUs were associated with a greater systemic inflammatory response. LBP showed the strongest discriminatory performance, but external validation is required before clinical application.

Serum Tumour Necrosis Factor-Alpha and Interleukin-4 as Immunological Indicators of hwp1-Confirmed Candida albicans Respiratory Tract Infection: A Case-Control Study from Wasit Province, Iraq

Introduction: Candida albicans is a critical-priority fungal pathogen and an increasingly recognised cause of respiratory tract infection in hospitalised and immunocompromised patients. Tumour necrosis factor-alpha (TNF-α) and interleukin-4 (IL-4) exert opposing influences on antifungal immunity, yet their combined beh...

Plasma lipopolysaccharide-binding protein (LBP), a proxy for intestinal permeability, relates to clinicopathological and immunological disease activity in inflammatory bowel disease patients at diagnosis and during disease course

Abstract Background and Aims Intestinal barrier healing is the treatment goal in ulcerative colitis (UC) and Crohn’s disease (CD). Despite this, systematic analyses of the relationship between intestinal permeability and clinicopathological and immunological disease activity at diagnosis and during follow-up are lackin...

Evaluation of GCF levels of leucine-rich alpha-2 glycoprotein and TNF-α in stage III periodontitis patients: A case–control study

Background Periodontitis results from interactions between oral biofilms and the host immune response, with acute-phase proteins indicating disease presence. This study measured leucine-rich alpha-2 glycoprotein (LRG) and tumor necrosis factor-alpha (TNF-α) levels before and after full-mouth disinfection. Their levels...

EVALUATION OF SERUM IL-6, IL-17, AND TNF- A AS POTENTIAL IMMUNOLOGICAL BIOMARKERS IN PATIENTS WITH VITILIGO

Background: Vitiligo is a chronic immune-mediated depigmenting disorder accompanied by both hyperactive inflammatory and autoimmunity pathways. Objective: To evaluate the serum levels of interleukin-6 (IL-6), interleukin-17 (IL-17), and tumor necrosis factor-A (TNF-A) in vitiligo patients, determine the correlation of...

The Role of Lipopolysaccharide-Binding Protein in Hepatitis B-Related Cirrhosis: Is There a Link to Portal Hypertension?

Background: Bacterial translocation plays a critical role in the pathogenesis of systemic inflammation in cirrhosis, particularly in cases related to chronic Hepatitis B infection. Lipopolysaccharide-binding protein (LBP) is a key acute-phase reactant that reflects exposure to endotoxins, yet its relationship with port...

Serum interleukin-38 is paradoxically elevated in rheumatoid arthritis and associated with disease activity and treatment intensity: a cross-sectional study

Interleukin-38 (IL-38), an anti-inflammatory IL-1 superfamily member, is paradoxically elevated in Rheumatoid Arthritis (RA). Whether this reflects pathogenic involvement or compensatory counter-regulation remains unresolved. Co-measuring Protein Tyrosine Phosphatase Non-Receptor (PTPN22) expression offers a unique opp...
