CTI may be a useful adjunct for preoperative risk stratification of POD in older surgical patients, but requires prospective validation.

Abstract

Postoperative delirium (POD) is a prevalent and serious complication in older surgical patients. The C-reactive protein-triglyceride-glucose index (CTI) is a recently developed composite marker that integrates both Insulin resistance (IR) and systemic inflammation. This study aimed to evaluate the association between CTI and POD in older surgical patients. Older surgical patients were retrospectively enrolled. The CTI was calculated and analyzed as both a continuous variable and a binary variable dichotomized at the optimal cut-off derived from receiver operating characteristic (ROC) analysis. Univariate and multivariable logistic regression models were constructed to examine the association between CTI and POD. Joint stratification by the TyG index and CRP was performed to delineate the synergistic contributions of IR and inflammation. A total of 10,125 patients were included in the final analysis. The receiver operating characteristic (ROC) curve analysis determined the optimal cut-off value for CTI to be 7.57. As a binary variable, high CTI conferred significantly elevated odds of POD compared with low CTI in fully adjusted model (OR = 1.60, 95% CI: 1.28-2.00, P < 0.001). As a continuous variable, each unit increment in CTI was associated with a 35% increased risk of POD after full adjustment (OR = 1.38, 95% CI: 1.21–1.56, P < 0.001). Joint stratification further confirmed that patients with concurrently high TyG and high CRP exhibited the highest POD incidence. CTI may be a useful adjunct for preoperative risk stratification of POD in older surgical patients, but requires prospective validation.

C-reactive protein–triglyceride–glucose index predicts acute pancreatitis severity: a retrospective cross-sectional study

Objective Early prediction of acute pancreatitis (AP) severity is essential for timely intervention and improved outcomes. The C-reactive protein–triglyceride–glucose index (CTI) is a simple, routine laboratory-derived biomarker. It integrates signals of inflammation and insulin resistance (IR) and has demonstrated pre...

The association of high-sensitivity C-reactive protein-triglyceride glucose index with functional outcome in elderly patients with acute ischemic stroke undergoing intravenous thrombolysis

Background and purpose High-sensitivity C-reactive protein-triglyceride-glucose index (CTI) is an innovative biomarker of insulin resistance and inflammation. The objective of this study was to explore the association of high-sensitivity CTI with functional outcome in elderly patients with acute ischemic stroke (AIS) u...

C-reactive protein-to-albumin ratio adds independent prognostic value to the clinical frailty scale in older emergency department patients: a multicenter cohort study

Older patients account for a growing share of emergency department (ED) visits with heterogeneous risk that conventional triage does not capture. The Clinical Frailty Scale (CFS) reflects baseline reserve and the C-reactive protein-to-albumin ratio (CAR) reflects acute inflammatory–nutritional stress. Whether com...

Nonlinear association of the C-reactive protein-triglyceride-glucose index with 28-day mortality in critically ill children: a retrospective cohort study.

BACKGROUND The C-reactive protein-triglyceride-glucose index (CTI) has shown prognostic value in adults, but its association with short-term mortality in critically ill children remains unclear. We investigated the association of CTI with 28-day mortality, including its nonlinear relationship and incremental predictive...

Association and predictive value of the C-reactive protein-triglyceride-glucose index in chronic kidney disease

Recent evidence has suggested a positive association between the C-reactive protein-triglyceride-glucose index (CTI) and chronic kidney disease (CKD), but its generalizability and prognostic utility remain to be fully established. In this study, we examined the CTI and CKD association in a nationally representative U...
