TL;DR

Higher HbA1c levels were independently associated with a more atherogenic lipid profile—higher TC and TG, and lower HDL-C—highlighting the importance of early metabolic assessment and comprehensive cardiovascular risk management.

Abstract

Background and objectives Type 2 diabetes mellitus (T2DM) is associated with lipid abnormalities that increase cardiovascular risk; however, evidence regarding this relationship at diagnosis is limited. This study aimed to examine the association between glycated hemoglobin (HbA1c) and lipid parameters at the time of T2DM diagnosis. Methods A retrospective cross-sectional study included 3,501 newly diagnosed T2DM patients treated within the Riojano Health Service between 2019 and 2025. Multiple linear regression was used to assess associations between HbA1c and lipid parameters. The study was approved by the Research Ethics Committee of La Rioja (CEImLAR, PI 778). Results HbA1c was positively associated with total cholesterol (TC) (β = 4.25, p < 0.001) and triglycerides (TG) (β = 15.65, p < 0.001) and negatively associated with high-density lipoprotein cholesterol (HDL-C) (β = −1.40, p < 0.001) in multiple linear regression adjusted for age, sex, and body mass index (BMI). No significant association was found between HbA1c and low-density lipoprotein cholesterol (LDL-C) (β = 1.67, p = 0.080). Conclusion At diagnosis, higher HbA1c levels were independently associated with a more atherogenic lipid profile—higher TC and TG, and lower HDL-C—highlighting the importance of early metabolic assessment and comprehensive cardiovascular risk management.

Evaluation of Glycated Hemoglobin and Lipid Profile in Type 2 Diabetes Mellitus: A Systematic Review

Background: Type 2 diabetes mellitus (T2DM) is frequently accompanied by dyslipidemia, both of which contribute independently and synergistically to cardiovascular risk. Glycated hemoglobin (HbA1c) reflects long-term glycemic control and has been proposed as an indirect marker of lipid abnormalities in T2DM. This syste...

Association between glycated hemoglobin and hematological parameters in patients with type 2 diabetes mellitus: a cross-sectional study

Highlights • HbA1c was associated with selected red blood cell indices in T2DM.• MCHC showed the most consistent adjusted association with HbA1c.• HbA1c-hematocrit associations differed between female and male patients.• Anemia prevalence varied significantly across glycemic control categories.• Findings support cautio...

Risk of type 2 diabetes mellitus associated with lipid-related metabolic indicators: a diabetes family cohort study in China

To evaluate the associations between lipid-related metabolic indicators and incident type 2 diabetes mellitus (T2DM). A diabetes family cohort study of 3,726 participants was conducted in Taizhou from May to October 2021 to track incident T2DM cases. Lipid metabolism indicators included triglycerides (TG),...

Association between uric acid to HDL cholesterol ratio and HbA1c in type 2 diabetes mellitus: a novel biomarker of glycemic control

The uric acid–to–high-density lipoprotein cholesterol ratio (UHR) has emerged as a potential metabolic marker reflecting insulin resistance and metabolic imbalance. However, evidence regarding its association with glycemic control in type 2 diabetes mellitus (T2DM) remains limited. This study aimed to evaluate the rela...

Association between glycated hemoglobin A1c and diabetic retinopathy in adults with type 2 diabetes mellitus residing at high altitudes

Aims To investigate the association between glycated hemoglobin A1c (HbA1c) and diabetic retinopathy (DR) in adults with type 2 diabetes mellitus (T2DM) at high altitude, and to explore potential effect modifiers. Methods This retrospective, hospital-based, cross-sectional study included 1040 adults with T2DM residing...

Distribution of Homocysteine Levels in Type 2 Diabetes Patients and Its Association with Urine Albumin Creatinine Ratio: A Cross-sectional Study

Introduction: Homocysteine (Hcy) is a metabolite whose level increases with age, in patients with Vitamin B12 deficiency, and is associated with increased risk of Cardiovascular Disease (CVD). Diabetes patients have been reported to have an increased prevalence of B12 deficiency and have a higher than baseline risk for...
