Abstract
Ovarian Hyperstimulation Syndrome (OHSS) and preeclampsia are well-documented complications of Assisted Reproductive Technology (ART) and ovulation induction. In patients with Polyendocrine Metabolic Ovarian Syndrome (PMOS) who are undergoing fertility treatment, these conditions may occur separately or sequentially. Their simultaneous occurrence after letrozole ovulation induction, a medication with a low OHSS is rarely documented. We report a rare case of OHSS and preeclampsia after ovulation induction with letrozole in a woman with polyendocrine metabolic ovarian syndrome. Case summary:-A 25-year-old woman with polyendocrine metabolic ovarian syndrome conceived following letrozole ovulation induction. She presented as an emergency at 30 weeks of gestation with abdominal pain, persistent headache, elevated blood pressures, ascites and significant bilateral ovarian enlargement. She was diagnosed with moderate OHSS and severe early onset preeclampsia. Management included hospital admission, fluid balance monitoring, prophylactic anticoagulation, and antihypertensive therapy up to 34 weeks. She underwent caesarean section delivery at 34 weeks.
Gonadotrophin releasing hormone agonist is as effective as human chorionic gonadotrophin in triggering ovulation and achieving pregnancy in PCOS patients, with the added clinical advantage of eliminating the risk of ovarian hyperstimulation syndrome.
Laroscopic ovarian drilling is a preferred therapeutic alternative, effective and safe compared to gonadotropins, reducing the risks of ovarian hyperstimulation and multiple pregnancies in anovulatory PCOS patients resistant to clomiphene.

We compared the risks of moderate-to-severe ovarian hyperstimulation syndrome (OHSS) and the probabilities of cumulative clinical pregnancy between non-overweight women with polycystic ovary syndrome (PCOS) who received early follicular-phase long-acting gonadotropin-releasing hormone (GnRH) agonist or GnRH antagon...

Background Polyendocrine Metabolic Ovarian Syndrome (PMOS) is a common endocrine disorder and a leading cause of anovulatory infertility. In women with PMOS, controlled ovarian stimulation (COS) often results in heterogeneous responses, with many oocytes of poor quality and lower fertilization rates. The combined use o...

Background: Ovarian hyperstimulation syndrome (OHSS) is a serious iatrogenic complication of controlled ovarian stimulation during in vitro fertilization (IVF). Several preventive strategies have been proposed to reduce its incidence and severity. Objectives: To evaluate the efficacy of prophylactic albumin combined wi...
