Abstract
Gallbladder cancer (GBC) is an aggressive malignancy where tumor-intrinsic genomic alterations fail to fully explain its rapid invasiveness and therapeutic resistance. Unlike more generic reviews focused broadly on biliary tract cancers or limited to isolated genomic profiling, this review provides an in-depth dissection of the GBC tumor microenvironment (TME) as an integrated tumor-immune-stromal-neural ecosystem. Key microenvironmental drivers are illuminated, including cancer-associated fibroblast remodeling, immunosuppressive myeloid reprogramming, T-cell dysfunction, and neuro-tumor crosstalk driving perineural invasion. Crucially, this review establishes a structured evidence hierarchy that rigorously distinguishes GBC-specific findings from broader biliary cohorts and tumor-agnostic data. By synthesizing multimodal computational frameworks and microenvironment-directed combination strategies, this review provides an actionable precision oncology roadmap to overcome therapeutic resistance and improve patient outcomes in GBC.
A Dynamic Precision Oncology (DPO) framework is proposed that extends conventional precision oncology beyond baseline molecular profiling by integrating longitudinal assessment of tumor genomics, circulating tumor DNA, CA19-9, imaging, radiomics, and clinical characteristics and summarizes emerging strategies for overc...
Pancreatic ductal adenocarcinoma (PDAC) remains one of the most treatment-refractory malignancies, with limited and often transient benefit from chemotherapy, targeted therapy, immunotherapy, and radiotherapy. Therapeutic resistance is increasingly recognized as an ecosystem-level process arising from reciprocal intera...

Colorectal cancer (CRC) remains one of the most frequently diagnosed malignancies worldwide, and colorectal cancer liver metastasis (CRLM) represents a major cause of CRC‐related mortality. The liver provides a highly specialized environment that can be remodeled into a metastatic niche, where anatomical structure, met...

Thyroid cancer (TC) and colorectal cancer are common malignancies of the endocrine and gastrointestinal systems, respectively, and continue to impose a substantial global disease burden. The tumor immune microenvironment (TIME) plays a central role in tumor initiation, progression, and therapeutic response; however, th...
Gynecological malignancies, encompassing ovarian cancer (OC), endometrial cancer (EC), and cervical cancer (CC), represent a group of aggressive neoplasms that pose substantial threats to women’s health worldwide. The complex tumor microenvironment (TME) plays a pivotal role in disease progression and therapeutic resis...

Despite breakthroughs in immune checkpoint inhibitors (ICIs) across various malignancies, prostate cancer (PCa), particularly metastatic castration-resistant prostate cancer (mCRPC), has shown limited responses to ICIs. This pervasive primary resistance is fundamentally driven by the canonically “immune-cold” and highl...
