TL;DR

A more uniform and less tapered aortic arch geometry was associated with higher right ventricular function and cardiac index following the Norwood procedure for HLHS.

Abstract

Background: The outcomes for infants with hypoplastic left heart syndrome (HLHS) have improved in recent years, partly due to advances in the Norwood procedure. An unobstructed aortic arch is important for maintaining right ventricular function. Previous studies have suggested that a highly tapered aortic arch may increase energy loss; however, the relationship between aortic arch geometry and right ventricular function remains unclear. Methods: This study was a retrospective single-center analysis. Results: During the study period, a total of 14 patients with HLHS who underwent the Norwood procedure and had contrast-enhanced computed tomography prior to the Glenn procedure were included. The median age at the Norwood procedure was 30 days (25-269), and the median weight was 3.0 kg (2.6-4.4). As an indicator of aortic arch tapering, we used the standard deviation of diameters measured at the ascending aorta, transverse arch, aortic isthmus, and descending aorta (Arch SD). There was a significant negative correlation between Arch SD and cardiac index. A significant negative correlation was also observed between Arch SD and right ventricular fractional area change. Conclusions: A more uniform and less tapered aortic arch geometry was associated with higher right ventricular function and cardiac index following the Norwood procedure for HLHS.

Quantitative CT Geometry of the Patent Ductus Arteriosus in Infants with Congenital Heart Disease: Association with Aortic Arch Obstruction or Hypoplasia

LDI was lower in patients with arch obstruction/hypoplasia than in those without these findings, and the association remained robust with a conservative definition restricted to coarctation or interrupted aortic arch.

Exploring the role of ascending aortic length to prevent type A acute aortic dissection

Background Stanford type A acute aortic dissection (AAD) often occurs before the given threshold for prophylactic surgery. This study investigates the role of the ascending aortic length (AAL) integrated to aortic height index (AHI) as an additional factor in the risk stratification of AAD. Thereby, the relationship be...

Relationship between bicuspid aortic valve phenotype, valvular function, and ascending aortic dimensions.

Background and aim of the study: Bicuspid aortic valve (BAV) is known to be associated with aortic valve dysfunction and ascending aorta (AA) dilatation. However, the relationship between BAV morphology and AA dimensions remains unclear. Thus, the study aim was to characterize the aortic valve function and AA dimension...

Temporal changes in dilated ascending aorta after transfemoral TAVR: a comparative analysis of bicuspid and tricuspid aortic valves

Background Transcatheter aortic valve replacement (TAVR) addresses valvular obstruction but leaves concomitant ascending aortic dilatation untreated. Whether bicuspid aortic valve (BAV) morphology inherently accelerates post-TAVR aortic expansion compared with tricuspid aortic valves (TAV) remains uncertain. Methods Th...

Preprocedural computed tomography angiography aortic root geometry and 1-year left ventricular reverse remodeling after transcatheter aortic valve replacement

Introduction and objectives Left ventricular reverse remodeling (LVRR) after transcatheter aortic valve replacement (TAVR) is associated with outcomes, but the relevance of aortic-root geometry remains uncertain. We examined whether the annular eccentricity index (EI) and sinus of Valsalva height on computed tomography...
