TL;DR

LDI was lower in patients with arch obstruction/hypoplasia than in those without these findings, and the association remained robust with a conservative definition restricted to coarctation or interrupted aortic arch.

Abstract

Background/Objectives: Quantitative patent ductus arteriosus (PDA) geometry in congenital heart disease (CHD), particularly in relation to aortic arch abnormalities, is incompletely characterized. We evaluated CT-derived PDA geometry. Methods: This retrospective exploratory single-center study included 168 infants with CHD and a PDA on cardiovascular CT. Aortic-end, pulmonary-end, and mid-duct diameters and ductal length were measured. The Ductal Length-to-Diameter Index (LDI), selected during exploratory analysis, was defined as ductal length divided by the mean end diameter. Seventy-three patients had CT-documented aortic arch obstruction or hypoplasia. Results: LDI was lower in patients with arch obstruction/hypoplasia than in those without these findings (2.04 [1.39–2.84] vs. 3.51 [2.23–4.89]; p < 0.001; rank-biserial r = −0.473) and remained associated after adjustment for age, weight, and sex (OR per +1 LDI, 0.534; 95% CI, 0.412–0.693; p < 0.001). The association remained robust with a conservative definition restricted to coarctation or interrupted aortic arch. LDI had an interobserver ICC of 0.843 (95% CI, 0.685–0.954), with wide limits of agreement (−1.98 to +2.45). Conclusions: CT-documented aortic arch obstruction or hypoplasia is associated with lower LDI. This exploratory geometric association warrants prospective external validation before clinical use.

Echo-Cardio Graphical Evaluation of Morphology of Bicuspid Aortic Valve and Associated Cardiac Malformations in Pediatric Patients

Introduction: Bicuspid aortic valve (BAV) occurs both as an isolated cardiac lesion and in association with congenital heart defects (CHD). Their aim was to identify the incidence and morphology of BAV in patients with coexisting CHD and complications compare to patients with isolated BAV. Methodology: The echocardiog...

Background: Tetralogy of Fallot with pulmonary atresia is a complex congenital heart defect frequently associated with major aortopulmonary collateral arteries (MAPCAs), which significantly affect pulmonary blood supply and surgical strategy. Objective: To assess the prevalence and morphological characteristics of MAPC...

Isolated Closure versus Mitral Valvuloplasty for Patent Ductus Arteriosus in Patients with Mitral Regurgitation: A Single-Center, Retrospective Study

: Background: Patent ductus arteriosus (PDA) can cause left-heart volume overload and functional mitral regurgitation (MR), although some patients have structural mitral abnormalities requiring repair. We compared MR and cardiac remodeling after isolated PDA closure and PDA closure combined with mitral valvuloplasty (M...

Computed tomography angiography of congenital left-sided obstructive heart lesions: a pictorial essay.

Congenital left-sided obstructive heart lesions (LSOLs) are a group of rare but serious congenital heart diseases. They encompass a spectrum of structural abnormalities that can obstruct blood flow at any point from the left atrium to the aortic arch. They are broadly divided into two main categories. The first is left...

The Heart of Fetal Growth Restriction: Congenital Heart Defects—An Analysis of Risk Factors and Morbidity Outcomes

Background: Congenital heart disease (CHD) co-occurring with fetal growth restriction (FGR) is clinically important but incompletely characterized. We assessed the proportion and subtype spectrum of CHD in term-born children with FGR, and factors associated with its presence and complexity. Methods: Retrospective analy...
