Bone Ring Grafts Versus Non-Resorbable Membrane GBR for Vertical Ridge Augmentation: A Prospective Cohort Study

Abstract

Purpose Vertical alveolar bone defects often require advanced augmentation techniques to enable successful implant placement. The aim of this study was to compare short-term vertical bone gain following two approaches: the bone-ring graft technique and guided bone regeneration (GBR) using a non-resorbable membrane in patients with vertical alveolar deficiencies. Materials and Methods A prospective cohort study was conducted involving 30 systemically healthy adult patients with posterior mandibular vertical bone defects measuring ≥4 mm. Participants were assigned to one of two treatment groups: Group A received the bone ring in conjunction with titanium implants and a resorbable collagen membrane; Group B was treated with allogeneic bone grafts and titanium-reinforced dense polytetrafluoroethylene (d-PTFE) membranes. Radiographic evaluations were performed at baseline, 1 week, 1 month, and 3 months postoperatively by cone-beam computed tomography (CBCT) and periapical radiographs. The primary outcome was vertical bone height gain. The data were analyzed using IBM SPSS Statistics Version 28.0. Results Group A demonstrated statistically significantly greater vertical bone gain at all postoperative intervals: 9.1 mm at 1 week, 10.6 mm at 1 month, and 9.7 mm at 3 months, compared to 7.8 mm, 7.0 mm, and 6.9 mm, respectively, in Group B (p < 0.0001). Plaque index scores were comparable between groups, with no statistically significant differences observed in postoperative swelling or infection rates. However, membrane exposure was statistically significantly more frequent in Group B at all follow-up visits (p ≤ 0.02). Conclusion The bone-ring graft technique was associated with greater vertical bone gain and a lower incidence of membrane exposure compared to GBR using a non-resorbable membrane. Further studies with longer follow-up are required to confirm these findings.

Cleft Alveolar Bone Grafting Using Mandibular Symphyseal Bone Graft and Resorbable Collagen Membrane: A CBCT-Based 3-Dimensional Morphometric Analysis.

ObjectiveIliac cancellous bone graft has been regarded as the gold standard for secondary alveolar bone grafting (SABG); however, it is associated with a high resorption rate and donor-site morbidity. This study aimed to evaluate the radiographic outcomes of SABG using a mandibular symphyseal bone graft combined with a double-layer resorbable collagen membrane through standardized 3-dimensional cone-beam computed tomography (CBCT)-based morphometric assessment.DesignRetrospective study.SettingPatients with unilateral cleft lip/palate (UCLP) or bilateral CL/P (BCLP) who underwent SABG at a single center.Patients and InterventionEighteen patients with UCLP/BCLP who underwent SABG using a mandibular symphyseal bone graft and a double-layer resorbable collagen membrane between 2022 and 2025.Main Outcome MeasuresPreoperative and 1.5-year postoperative CBCT-based 3-dimensional linear morphometric measurements, including cleft width reduction and postoperative bone bridge thickness.ResultsAt 1.5 years postoperatively, cleft width significantly reduced at all measurement levels (P < .001). The mean postoperative bone bridge thickness was 5.97 ± 2.02 mm at the mid-root level and 6.62 ± 2.50 mm at the apical level.ConclusionsMandibular symphyseal bone grafting combined with a double-layer resorbable collagen membrane yielded favorable radiographic outcomes, with substantial reduction of alveolar cleft defects and satisfactory bone bridge formation in this retrospective cohort. This technique may be a viable treatment option for selected patients. However, prospective controlled studies with larger cohorts and longer follow-up are needed to confirm its comparative effectiveness and long-term stability.

H-PRF Bone Block Versus Sausage Technique for Horizontal Ridge Augmentation: A Retrospective Study.

OBJECTIVES Adequate horizontal bone width is essential for successful implant placement and long-term stability. This study aimed to compare the outcomes of two techniques for horizontal alveolar ridge augmentation: the horizontal platelet-rich fibrin (H-PRF) bone block technique and the Sausage technique. MATERIALS AND METHODS Fifty patients, presenting 63 tooth sites, were enrolled between 2023 and 2025. Twenty five patients were treated with H-PRF bone block technique (H-PRF group), while 25 patients were treated using the "Sausage" technique (Sausage group) for horizontal bone augmentation. The bone width gain, bone stability rate, and bone volume change at 6 months after surgery were quantified by measurements through cone beam computed tomography (CBCT) scans. Furthermore, the correlation between bone width gain, bone stability rate, and gender, defect site, and bone defect type was analyzed. The complication rates were also recorded. RESULTS The H-PRF group demonstrated significantly greater horizontal bone gain at the crestal level compared with the Sausage group (0 mm level: 2.85 ± 4.05 mm vs. 0.79 ± 2.35 mm; p = 0.043), while no significant differences were observed at 2, 4, 6, 8 mm levels (p > 0.05). The bone stability rate was 63.21% ± 27.67% for the H-PRF group and 63.88% ± 16.64% for the Sausage group (p = 0.791). Both bone width gain and bone stability rate were not associated with gender or defect site. However, defect type showed a significant association with horizontal bone width gain at the most apical levels (6 and 8 mm). CONCLUSIONS The H-PRF bone block technique achieved comparable bone augmentation outcomes to the Sausage technique; this technique represents a promising approach that may be considered for horizontal bone augmentation.

Autogenous Bone Graft for Horizontal Ridge Augmentation Associated with Simultaneous Implant Placement

The goal of this study was to evaluate the efficacy of autogenous bone grafts with and without recombinant human bone morphogenetic protein-2 for rehabilitation of narrow alveolar ridge width by dental implants. Twelve patients (2 males and 10 females), ranging from 36 to 60 years and a mean age of 47.08 years, were divided into two study groups. One received autogenous bone graft (A1) and the other group autogenous bone graft with rhBMP-2 using a collagen membrane (A2). All patients were clinically evaluated and received a CBCT for an accurate measurement of the alveolar ridge width, height, and after six months postoperatively. The mean difference in primary and secondary alveolar bone width (after 6 months) in group A1 was 4.14 mm, with a statistically significant difference (p < 0.05). Also, the mean difference in primary and secondary (after 6 months) alveolar bone width in group A2 was 5.14 mm, with a significant difference between them (P-value 0.00). The addition of rhBMP-2 increased the extent of bone width regeneration compared with the autogenous bone graft used alone. Both study groups showed a statistically significant increase in bone width at 6 months, with the highest increase observed in group A2 (ABG + rhBMP-2).

Comparative Effectiveness of Vertical Ridge Augmentation Techniques for Dental Implant Treatment: A Systematic Review

While DO provides the greatest vertical bone gain, the GBR techniques might be a more clinically practical option, however, future well-planned RCTs to identify effective vertical bone augmentation techniques are still warranted.
