TL;DR

NS1-based serology revealed a markedly underestimated TBEV infection burden and may improve surveillance, identification of emerging risk areas, and targeted vaccination strategies.

Abstract

Objectives

Tick-borne encephalitis virus (TBEV) is a major vector-borne pathogen in Europe, but its true infection burden remains unclear due to the high proportion of asymptomatic infections and the inability of conventional serology to distinguish vaccination- from infection-induced antibodies. We applied a non-structural protein 1 (NS1)-based serological approach to estimate TBEV seroprevalence, incidence, and manifestation index in eastern Germany (Saxony and Berlin/Brandenburg).

Methods

A total of 1,391 anonymized blood donor sera (18-72 years), collected between May and September 2021, were analyzed using commercial TBEV IgG ELISA, an in-house NS1 IgG ELISA, and micro-neutralization testing.

Results

Overall, 39.8% of samples were TBEV IgG-positive, indicating vaccination- and/or infection-induced immunity. NS1 antibodies, reflecting past or recent infection, were detected in 1.3% of donors. Estimated vaccination coverage was 38.5%, with higher coverage in Saxony than in Berlin-Brandenburg (45.4% vs. 32.0%). NS1-based analyses revealed a mean annual incidence of 64.7 cases per 100,000 inhabitants, approximately 294-fold higher than notification-based estimates, with a 1000-fold difference in Berlin/Brandenburg. The overall manifestation index was 0.34%, indicating that >99% of infections remain asymptomatic or undiagnosed. Despite higher reported incidence in Saxony, NS1-based estimates indicated substantial infection activity in Berlin/Brandenburg.

Conclusion

NS1-based serology revealed a markedly underestimated TBEV infection burden and may improve surveillance, identification of emerging risk areas, and targeted vaccination strategies.

Seroprevalence of Japanese encephalitis virus and other flavivirus-specific antibodies among Australian blood donors following a novel epidemic spread of Japanese encephalitis virus: a cross-sectional study

Abstract Objectives In 2022, Japanese encephalitis virus (JEV) was detected in humans and animals across mainland Australia. We examined the prevalence of JEV and endemic flaviviruses, Murray Valley encephalitis virus (MVEV) and Kunjin virus (KUNV) antibodies in blood donors in different risk geographic areas across so...

Surveillance of tick-borne encephalitis virus and Tribeč Virus in Ixodes ricinus ticks from Slovakia

Findings suggest possible co-circulation of TBEV and TRBV in I. ricinus populations in selected sites in Slovakia and raise the possibility of co-infections, highlighting the importance of continued surveillance of tick-borne viruses.

Serological evidence of subclinical Usutu virus exposure among West Nile virus-seropositive individuals in Israel.

BACKGROUND Usutu virus (USUV) is a mosquito-borne orthoflavivirus closely related to West Nile virus (WNV), with confirmed circulation in mosquitoes and horses in Israel but no reported human exposure. OBJECTIVES To determine whether USUV circulates in humans in Israel and to assess its contribution to serological cr...

TICK-BORNE ENCEPHALITIS VIRUS PREVALENCE IN EUROPEAN TICK POPULATIONS: A GEOGRAPHIC PERSPECTIVE

This systematic review investigates TBEV prevalence and expansion across 26 countries to evaluate shifting transmission risks and the emergence of new microfoci, finding that TBEV distribution is highly dynamic, driven by climate-mediated range shifts and the silent establishment of new microfoci.

A multiplex serological survey highlights differences in population immunity to mosquito-borne diseases in Vanuatu, Pacific region, 2023.

OBJECTIVES Mosquito-borne pathogens are major emerging diseases and a growing public health concern in the Pacific, with occurrences linked to global increases, climate change and human mobility. To support risk assessment of arboviruses and malaria in Vanuatu, a seroprevalence survey was conducted on Espiritu Santo Is...

Rates of severe acute respiratory syndrome coronavirus 2 infection in a highly exposed South African blood donor cohort, 2022-2023.

BACKGROUND AND OBJECTIVES A cohort of South African repeat blood donors was established to leverage the repeated presentation of blood donors for longitudinal serological surveillance of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infections and reinfection incidence in a highly exposed population.
