TL;DR

This case underscores the importance of recognizing atypical patterns of 99mTc-PSMA uptake to improve diagnostic accuracy, avoid misinterpretation, and ensure appropriate patient management.

Abstract

A 62-year-old man with newly diagnosed prostate adenocarcinoma underwent initial staging with 99mTc-PSMA and 99mTc-MDP whole-body scans. The bone scan revealed a small zone of tracer activity superior to the left kidney. Subsequent 99mTc-PSMA SPECT/CT revealed a fluid density collection in the left perinephric and pararenal space, extending from the left subdiaphragmatic area to the pelvic inlet. PSMA tracer activity was seen around the left kidney. Contrast-enhanced CT confirmed a perinephric fluid collection consistent with urinoma. This case underscores the importance of recognizing atypical patterns of 99mTc-PSMA uptake to improve diagnostic accuracy, avoid misinterpretation, and ensure appropriate patient management.

Utility of 99mTc-FAPI-46 SPECT/CT in Restaging Hepatocellular Carcinoma: A Case Report.

We present a 54-year-old man with hepatocellular carcinoma (HCC) presenting with diffuse bone pain 6 months postradiofrequency ablation. 99mTc-FAPI-46 SPECT/CT was performed to evaluate disease extent, confirming locally active primary HCC and revealing widespread FAPI-avid bone metastases. Considering the low performa...

[68Ga]Ga-PSMA-11 PET/CT Detection of Perineural Invasion in Prostate Cancer.

The case of a 78-y-old man with previously treated high-risk PCa who developed biochemical recurrence is presented, where [68Ga]Ga-PSMA-11 PET/CT demonstrated tumor spread along sacral nerve roots and the sciatic nerve, suggestive of perineural invasion.

From negative bone scintigraphy to oncologic superscan within five months: Explosive PSA kinetics in grade group 5 prostate adenocarcinoma

A 78-year-old man presented with a prostate-specific antigen (PSA) level of 35 ng/mL, a PI-RADS 5 lesion on multiparametric prostate MRI, and negative whole-body bone scintigraphy. Within five months, PSA increased to 1244 ng/mL despite minimal radiological progression of the primary tumour. Repeat MRI demonstrated mul...

Large Calyceal Diverticulum Mimicking a Solid Renal Mass in a Patient Undergoing 68Ga-DOTATATE PET/CT.

A 41-year-old man with grade 1, well-differentiated neuroendocrine tumor of the terminal ileum underwent initial staging with 68Ga-DOTATATE PET/CT and evaluation with contrast-enhanced MRI demonstrated a finding consistent with a large calyceal diverticulum.

The "Scintigraphic Flip": A Visual Warning Sign of Impending Hematologic Toxicity.

We report a 55-year-old man with metastatic castration-resistant prostate cancer who developed a dramatic "scintigraphic flip" after the second cycle of ¹⁷⁷Lu-PSMA therapy. Post-therapy SPECT and follow-up ¹⁸F-PSMA PET/CT showed increased diffuse marrow uptake with paradoxically reduced salivary-gland and liver activit...
