Sindroma Down: Dari Replikasi DNA hingga Fenotipe Klinis — Kajian Biologi Sel, Genetika, dan Embriologi Trisomi 21

Down syndrome and intellectual disability research

Abstract

Abstract Background. Down syndrome (DS) is the most common viable autosomal aneuploidy and the leading genetic cause of intellectual disability. About 95% of cases are free trisomy 21, arising from a single cytological event: failed segregation of chromosome 21 during gametogenesis or early zygotic division. Aim. This review traces the full causal chain of DS, from pre-meiotic DNA replication, chromosome and sister-chromatid behaviour in meiosis I and II, and nondisjunction in oogenesis and spermatogenesis, to the trisomic zygote, gastrulation into ectoderm, mesoderm and endoderm, and the molecular mechanisms that generate the clinical phenotype. Results. Roughly 90% of extra chromosomes 21 are maternal, mostly from errors in maternal meiosis I. Risk rises steeply with maternal age because oocyte cohesin (REC8–SMC1β) is loaded before birth and is not renewed during decades of dictyate arrest. Unfavourable recombination patterns, a permissive spindle assembly checkpoint and premature separation of sister chromatids compound this risk. A 1.5-fold dosage of more than 200 protein-coding HSA21 genes drives genome-wide transcriptomic dysregulation, chronic interferonopathy, altered DYRK1A/RCAN1–NFAT and Sonic hedgehog signalling, oxidative stress and APP-driven amyloid load. These act on derivatives of all three germ layers, producing intellectual disability, congenital heart disease, gastrointestinal malformations, haematopoietic disorders and early-onset Alzheimer disease. Conclusion. DS shows how one chromosome segregation error is translated, through developmental biology, into a multisystem phenotype. This cross-level understanding underpins screening, genetic counselling and pathway-targeted therapies.
