Abstract
Despite the success of statins, many patients with dyslipidaemia continue to have residual cardiovascular risk because of persistent low-density lipoprotein cholesterol (LDL-C) elevation, statin intolerance, hypertriglyceridaemia, or elevated lipoprotein(a) [Lp(a)]. Recent advances in molecular biology have led to the development of targeted lipid-lowering therapies, including proprotein convertase subtilisin/ kexin type 9 (PCSK9) inhibitors, adenosine triphosphate-citrate lyase (ACL) inhibitors, apolipoprotein C-III (ApoC-III) inhibitors, angiopoietin-like protein 3 (ANGPTL3) inhibitors, Lp(a)-lowering agents, and gene- editing technologies. This review summarises the mechanisms of action, landmark clinical trials, lipid- lowering efficacy, safety, and current clinical applications of these novel agents, with emphasis on therapies introduced since 2018. Collectively, these agents have expanded treatment options for patients with familial dyslipidaemia, severe hypertriglyceridaemia, and persistent residual cardiovascular risk while paving the way for precision medicine. As ongoing cardiovascular outcome studies and gene-editing technologies mature, these advances are expected to reshape the future of dyslipidaemia management and cardiovascular prevention.
Inclisiran may represent another milestone in the treatment of dyslipidemia, and the associated mechanism of action, pharmacology, clinical trial data, efficacy, safety, use in special populations, and adverse effects are comprehensively discussed.
