Abstract
ABSTRACT The 2026 Asia Pacific Heart Rhythm Society (APHRS) Guidelines represent the first comprehensive APHRS guidelines dedicated to the management of atrial fibrillation (AF) and comprise four parts covering epidemiology, detection/screening and integrated care; stroke prevention; rate and rhythm control; and lifestyle risk factor and comorbidity management. Part II focuses on stroke prevention in AF, integrating contemporary global evidence with data and clinical considerations specific to the Asia‐Pacific region. It provides evidence‐based and practical recommendations for stroke and bleeding risk assessment, oral anticoagulation, selection and appropriate dosing of direct oral anticoagulants, management of patients with special or complex clinical characteristics, and left atrial appendage occlusion. Particular attention is given to issues (and evidence) highly relevant to Asia‐Pacific populations, including the evolving risk of ischemic stroke, appropriate anticoagulant dosing, renal function assessment, and balancing thromboembolic and bleeding risks. These recommendations aim to support individualized clinical decision‐making, promote appropriate use of stroke‐prevention strategies, and improve outcomes for patients with AF across the Asia‐Pacific region.

ABSTRACT Atrial fibrillation (AF) is closely associated with comorbidities and lifestyle factors that influence the development, progression, and outcomes. Here the focus is on guidelines for the systematic management of hypertension, heart failure, coronary and structural heart disease, diabetes, dyslipidemia, cerebro...

Atrial fibrillation (AF) is one of the most common arrhythmias in clinical practice, significantly increasing the risk of adverse events such as stroke and heart failure. Rhythm control and stroke prevention are important components of integrated AF management. The combined catheter ablation and left atrial appendage o...
Atrial fibrillation (AF) is a major risk factor for acute ischemic stroke (AIS), and oral anticoagulation is the cornerstone of secondary prevention. The optimal timing of anticoagulation initiation remains controversial because of the need to balance the risks of early ischemic recurrence and hemorrhagic transformatio...

BACKGROUND
The 2023 AHA/ACC/HRS guidelines recommend against peri-procedural bridging of warfarin in patients with atrial fibrillation (AF) based solely on CHA₂DS₂-VASc score without additional thromboembolic (TE) risk factors. At our institution, decisions regarding bridging in moderate-to-high TE risk AF patients who...

Atrial fibrillation (AF) is the most common sustained cardiac arrhythmia encountered in clinical practice and represents a major global health burden due to its association with ischemic stroke, systemic thromboembolism, heart failure, hospitalization, and increased mortality. The prevalence of AF continues to rise bec...
